Le projet a été mis en place pour répondre aux besoins de prise en charge sanitaire et sociale des populations atteintes de handicap visuel qui habitent en territoire transfrontalier. En particulier, les associations d’usagers et les partenaires du projet ont pu constater deux problématiques phares : l’éloignement des populations des programmes de dépistage visuel et des services de rééducation. la faible densité de services spécialisés en zones rurales de montagne. Le projet se donne 6 objectifs stratégiques : Apporter une réponse de proximité dans un désert médical et permettre l’accès aux soins ophtalmologiques; Permettre le dépistage précoce pour éviter les frais des prises en charge plus lourds à terme; Contribuer au bien-vivre dans les territoires reculés par la mise en place d’activités sociales et culturelles; Contribuer au maintien des populations de tout âge dans les territoires transfrontaliers; Sensibiliser les institutionnels de la politique de santé publique sur l’importance des services de proximité; Proposer un modèle de coopération décloisonné pour une prise en charge pluridisciplinaire médico-social et environnemental basé sur les recommandations de l’OMS (Organisation mondiale de la Santé) et proposer un modèle d’intervention plus économique. Il progetto è stato creato per rispondere alle esigenze di assistenza sanitaria e sociale delle persone ipovedenti che vivono in aree transfrontaliere. In particolare, le associazioni di utenti e i partner del progetto hanno constatato due problematiche chiave: l'allontanamento della popolazione da programmi di screening visivo e servizi di riabilitazione. la bassa densità di servizi specializzati nelle zone rurali di montagna. Il progetto ha 6 obiettivi strategici: Fornire una risposta locale in un deserto medico e consentire l'accesso alle cure oculistiche; Permettere il rilevamento precoce per evitare costi di gestione più elevati nel tempo; Contribuire al "vivere bene" in aree remote creando attività sociali e culturali; Contribuire al mantenimento di popolazioni di ogni fascia d'età nei territori transfrontalieri; Rendere consapevole la politica istituzionale che si occupa di sanità pubblica dell'importanza di servizi locali; Proporre un modello di cooperazione aperto per la gestione multidisciplinare medico-sociale e ambientale basato sulle raccomandazioni dell'OMS (Organizzazione Mondiale della Sanità) e proporre un modello di intervento più economico. Il progetto è organizzato attorno a 4 attività tematiche: La creazione di un osservatorio epidemiologico transfrontaliero sulla sanità visuale. Svillupare delle nuove competenze tramite l'incontro tra professionisti della salute e del medico-sociale, in grado di influire sui protocolli nazionali in vigore. Sperimentazione di nuovi servizi sanitari locali. Organizzazione di un panel di attività di sensibilizzazione e promozione della salute visiva.

Description (EN):

The project has been thought to answer to needs of sanitarian and social taking over of populations suffering of visual disabilities in cross-border territories. In particular, users’ associations and project partners have underline two main problematics: • The remoteness of people of visual tests programs and re-education services. • The weak density of specialised services in rural mountain areas. The project has 6 objectives: 1. Bring an answer of proximity un a medical desert and allow the access to ophthalmologic cares. 2. Give the possibility to do an early ophthalmological test in order to avoid taking over costs, bigger on the long term. 3. Contribute to the well-being in remoted territories through the development of social and cultural activities. 4. Contribute to the maintenance of populations of all ages in cross-border territories. 5. Raising awareness of Public Health policy institutions on the importance of proximity services. 6. Purpose a model of decompartmentalized cooperation to a multidisciplinary health, social and environmental taking over based on WHO (World Health Organisation) recommendations and purpose a more economic model of intervention. The project is organized over 4 thematic activities: 1. The creation of a cross-border epidemic observatory on visual health. 2. Develop new skills thorough the meeting of professionals of sanitarian, social, and medico-social sector and create a common procedure of intervention, in grade to impact national process in force. 3. The experimentation of new proximity services of sanitarian taking over. 4. The organisation of a panel of activities focused on promotion and the raising of awareness toward visual health.

Expected Results (FR):

L’originalité du projet repose sur un nouveau modèle organisationnel du parcours de santé basé sur la coopération des acteurs du sanitaire, du social, du culturel et de l'institutionnel. En matière de promotion de la santé visuelle, 1 200 enfants ont été mobilisés lors de la journée « Regarddons Ensemble » organisée par l’APEDV (Association des parents d'enfants déficients visuels). Le projet prévoit, en termes de : Coordination et gestion du projet. • la création d’un espace collaboratif de travail. • 9 rapports techniques. • un outil de gestion de projet (Gantt). Communication sur le projet. un site internet. 2 clips vidéo. Constitution d’un Observatoire et Echange de bonnes pratiques. • une étude et analyse des besoins par la collecte de données épidémiologiques. • l'échange de bonnes pratiques professionnelles de visites néonatales et de rééducation des adultes et personnes âgées. Expérimentation de services de proximité et campagne de sensibilisation. • la mise en circulation opérationnelle du service de télé ophtalmologie et rééducation et service d’aide à domicile spécialisé. • des campagnes de sensibilisation au dépistage et à la prévention (écoles, adultes, personnes âgées, professionnels de la santé, institutions publiques en charge de la santé publique). Le projet vise à produire: • Une réduction des temps de prise en charge pour les personnes atteintes de déficience visuelle. • L'intégration de nouveaux protocoles d’intervention dans les pratiques professionnelles en vigueur de part et d’autre de la frontière. • L'augmentation du niveau de qualité de vie des sujets souffrant de handicap visuel (suivi de la rééducation à domicile et dans les centres de rééducation). • Le renforcement et proximité d’une assistance socio-sanitaire et éducative (personnes déficientes visuelles, entourage familiale, cadre périscolaire, professionnels).

Expected Results (IT):

L'originalità del progetto si basa su un nuovo modello organizzativo del percorso all'assistenza sanitaria basato sulla cooperazione degli attori sanitari, sociali, culturali e istituzionali. In termini di promozione della salute visiva, 1 200 bambini sono stati mobilitati durante la giornata "Regarddons Ensemble" (Guardiamo insieme) organizzata dall'APEDV (Associazione dei genitori di bambini ipovedenti). Il progetto prevede, in termini di: Coordinamento e gestione del progetto. la creazione di uno spazio di lavoro collaborativo. 9 rapporti tecnici. uno strumento di gestione di progetto (Gantt). Comunicazione sul progetto. un sito web. 2 video clip. Costituzione di un osservatorio e scambio di buone pratiche. uno studio e analisi di bisogni attraverso la raccolta di dati epidemiologici. lo scambio di buone pratiche professionali di visite neonatali e riabilitazione di adulti e anziani. Sperimentazione di servizi locali e campagna di sensibilizzazione. la diffusione operativa del servizio di teleoftalmologia e riabilitazione e del servizio di aiuto domiciliare specializzato. delle campagne di sensibilizzazione su screening e prevenzione (scuole, adulti, anziani, operatori sanitari, istituzioni pubbliche preposte alla salute pubblica). Impatto: Il progetto mira a produrre: Una riduzione dei tempi di cura per le persone con disabilità visive. L'integrazione di nuovi protocolli di intervento nelle pratiche professionali in vigore su entrambi i lati della frontiera. Il miglioramento della qualità della vita dei soggetti che soffrono di disabilità visiva (follow-up della riabilitazione a casa e nei centri di riabilitazione). Il rafforzamento e prossimità di un'assistenza socio-sanitaria ed educativa (persone ipovedenti, circolo familiare, ambiente extrascolastico, professionisti).

Expected Results (EN):

The originality of the project is based on a new organisational model of health paths based on the cooperation of actors of various sectors: sanitarian, social, cultural and institutional. Regarding the promotion of visual health, 1 200 children has been mobilized during the “Looking Together” Day organised by l’APEDV (Association of Parents having among visually children). The project has different objectives: Coordination and project management: • Creation of a collaborative working space. • 9 Technical Reports. • An instrument of project management (Gantt). Communication of the project: • A website. • 2 Video clips. Constitution of an Observatory and Exchanges of good practices. • Study and Analyses of needs by the collect of epidemiological data. • Exchanges of good professional practices in neonatal visits and re-education of adults and elders. Experimentation of proximity services and raising awareness campaigns: • Operational Development of a tele-ophthalmologic consultation and re-education and specialised home-help services. • Campaigns of raising awareness of visual tests and prevention (schools, adults, elders, professionals of Health sector, public institutions in charge of Public Health). The project aims to product: • A reduction of taking over’s times for people having a visual deficiency. • Integration of new intervention process in professional manners in force on both sides of the border. • Augmentation of life quality level of people suffering of a visual handicap (following of a re-education at the domicile but also in re-education centres). • Enforcement and proximity of a socio-sanitarian assistance and educative (persons having a visual deficiency, family, professionals) .
