Mismi est le projet transfrontalier qui réunit l'Italie et la France dans le but de développer des services sociaux et de santé de proximité intégrés qui contribuent à la lutte contre le dépeuplement des zones de montagne et à réduire les inégalités de santé, notamment par la prévention et le suivi de l'état de santé des personnes en particulier avec des maladies chroniques. Le projet repose sur deux piliers: l'autonomisation des personnes et des communautés du territoire. l'utilisation de nouvelles technologies telles que leviers d'innovation, d'amélioration de la qualité et de l'accès aux services et la réduction de l'isolement. Dans MisMi, les technologies sont considérées comme efficaces et durables dans la mesure où, simultanément, elles impliquent correctement les soignants et les réseaux sociaux. La coopération territoriale France-Italie est considérée comme le principal outil d'expérimentation d'un modèle intégré de services sanitaires et sociaux performants car proche des besoins de la personne et de son contexte. L'analyse des besoins, la comparaison et la discussion entre les spécificités des réalités italiennes et françaises se feront notamment au cours des formations, entendues comme plate-forme de démarrage du projet et d'activation des acteurs impliqués.

MisMi è il progetto transfrontaliero che riunisce ltalia e Francia con l’obiettivo di sviluppare servizi sociali e sanitari integrati di prossimità che contribuiscano a contrastare lo spopolamento delle zone di montagna e a ridurre le disuguaglianze di salute, in particolare attraverso la prevenzione e il monitoraggio dello stato di salute delle persone soprattutto con malattie croniche. Il progetto si basa su due pilastri: l’empowerment delle persone e delle comunità del territorio. l’utilizzo delle nuove tecnologie come leve per l’innovazione, il miglioramento della qualità e dell’accesso ai servizi e la riduzione dell’isolamento. In MisMi, infatti, le tecnologie sono considerate efficaci e sostenibili nella misura in cui, parallelamente, si coinvolgono correttamente care-givers e reti sociali. La cooperazione territoriale Italia-Francia è considerata lo strumento principale per sperimentare un modello integrato di servizi sia sanitari che sociali efficaci perché vicini ai bisogni della persona e del suo contesto. L’analisi dei bisogni, il confronto e la discussione tra le specificità delle realtà italiane e francesi avverrà soprattutto durante le sessioni della formazione, intesa come una piattaforma di start-up del progetto e di attivazione degli operatori coinvolti.

Description (EN):

Mismi is a cross-border project which bring together France and Italy in the scope to develop social and health services of proximity integrated which contributes to the fight against the depopulation of Mountain areas and to reduce health inequality especially for prevention and the following of people health state especially with chronically sick. The project is based on two pillars: 1. The autonomation of people and territorial communities 2. Use of new technologies in order to enhance innovation, amelioration of quality and access to services, reduction of isolation. With MisMi, technologies are considered as efficient and sustainable because, in the same time, they imply correctly carer and social medias. Territorial cooperation between France and Italy is considered as the principal instrument of experimentation in an integrated model of sanitarian and social performant services because they are close to people and of the context. The analyse of needs, the comparison and the discussion between specificities of Italian and French realities will be especially during formations, looked ad a platform of starting of the project and activation of implied actors. Information added on 2020-05-29, regarding the mitigation of the effects of COVID-19 (the coronavirus pandemic that started in 2019) (English language only): The project is about linking people in need with caregivers and the community using technology, thus contributing to fight people’s isolation and social and health inequalities. This project is even more valuable in the current context of COVID-19 spread as citizens can benefit from tele-medicine and two nurses who are helping on the ground in hospitals and patients’ home. Expected Results (FR):

La gouvernance et la communication soutiennent les activités du projet en valorisant les aspects transfrontaliers. L'analyse d'impact et l'attention particulière au développement des réseaux sociaux constituent la valeur ajoutée du projet. En France comme en Italie, MisMi s'intègre aux formes de programmation existantes (Contrat Local de Santé en Tarentaise et Plan de Zone en Vallée d'Aoste) et l'utilisation des financements vise principalement à construire un modèle durable à long terme. Pour les services sociaux, avec l'animateur de communauté et les équipes multiprofessionnelles seront identifiés les besoins de la population, soutenus les soignants et mis en œuvre des projets de promotion de la santé. Les services de santé de proximité sont basés sur la médecine d'initiative et sur la preuve de l'efficacité des interventions se déroulant dans les «lieux de vie» plutôt que dans les «lieux de la maladie». Les besoins de santé sont interceptés de manière proactive par les professionnels de la santé, en particulier par les infirmières de famille et de communauté (IFEC) et l'intervention favorise l'activation des ressources individuelles nécessaires au maintien du bien-être. Après les personnes, les outils: ainsi que l'autonomisation des personnes et des communautés, MisMi entend contrer le dépeuplement des zones de montagne à travers des technologies innovantes. Avec la téléconsultation, la télésurveillance et la télémédecine la technologie, avec des applications inclusives et économiquement durables, est au service des citoyens pour améliorer la qualité des services de santé.

La governance e la comunicazione supportano le attività del progetto valorizzandone gli aspetti transfrontalieri. Ne costituiscono valore aggiunto, la valutazione d’impatto e un’attenzione particolare allo sviluppo delle reti sociali. In Francia come in Italia, MisMi si integra con le forme di programmazione già attive (Contrat Local de Santé in Tarantaise e Piano di Zona in Valle d’Aosta) e l’uso dei finanziamenti mira soprattutto a costruire un modello sostenibile nel lungo periodo. Per i servizi sociali, con l’animatore di comunità e équipes multiprofessionali saranno rilevati i bisogni della popolazione, dato sostegno ai care givers e implementati progetti di promozione della salute. I servizi sanitari di prossimità si basano sulla medicina d’iniziativa e sull’evidenza dell’efficacia degli interventi che avvengono nei “luoghi di vita” piuttosto che nei “luoghi della malattia”. Il bisogno sanitario è intercettato in modo proattivo dai professionisti sanitari, in particolare dall’infermiere di famiglia e di comunità (IFeC) e l’intervento è tale da favorire l’attivazione delle risorse individuali necessarie per il mantenimento del benessere. Dopo le persone, gli strumenti: oltre che con l’empowerment delle persone e delle comunità, MisMi intende contrastare lo spopolamento delle zone di montagna attraverso tecnologie innovative. Con il teleconsulto, il telemonitoraggio e la telefisioterapia la tecnologia è al servizio dei cittadini, con applicazioni inclusive e economicamente sostenibile per migliorare la qualità dei servizi sanitari.

Expected Results (EN):

The governance and the communication sustain activities of the project valorising cross-border aspects. The impact analyses and the particular attention to social medias development constitute an added value to the project. In France, as in Italy, MisMi is integrated to pre-existing type of program (Local Contract of Health in Tarentaise and Zone Map in Aoste Valley) and the utilisations of funds aims specially to build a sustainable model for the long term. For social services, with the animator of community and multiprofessional teams will be identified needs of the population sustain by cares and develop projects of Health promotion. Proximity health services are based on the medicine of initiative and on proof of efficiency of interventions taking place in “places of life” more than in “Places for sick people”. Health needs are intercepted in a proactive manner by health professionals, especially by nurses of family and community and the intervention promote the activation of individual resources needed to the conservation of weel-being. In term of instruments, MisMi would like to fight against the loose of population in mountain areas thorough innovative technologies. With the teleconsultation, the telemonitoring and the telemedicine, technology is at the service of citizens to ameliorate the quality of health services.
