Description (EN): Prosthetic heart valves are the sole effective treatment in patients with valvular disease. Valve replacements in OECD countries is projected to be 850.000/year by 2050. Present prosthetic valves, while hemodynamically effective, are far from optimal due to progressive structural deterioration of tissue valves or chronic anticoagulation for mechanical valves, causing major constraints on quality of life. Valve-related problems lead to reoperation or death in 50-60% of patients within 10 years of prosthetic valve implantation. An ideal valve prosthesis would eliminate these issues. Polymeric heart valves fabricated from advanced polymeric materials offer the potential of durability and hemocompatibility. Clinical transferability of polymeric valves has been hampered by the occurrence of in vivo calcification, degradation and thrombosis. This programme will develop new polyurethanes to improve anti-thrombotic and anti-inflammatory properties and long-term performance of polymeric valves. With automated 3D printing, a precise and personalized leaflet geometric pattern that matches the heart valve dimensions will be achieved. Developing low cost, low toxicity materials with excellent biocompatibility and mechanical properties could highly valuable for broader biomedical applications.
Objectives:
1) Provide modified surface polyurethane polymers for prosthetic valve materials coating, with excellent mechanical properties, longer durability and biocompatibility. 2)Provide an individualized approach to patients based on patient-specific 3D valvular modelling. 3) Manufacture a precise leaflet geometric pattern that matches the heart valve dimensions with the use of 3D valvular models and 3D printing technology. 4) Provide high-end procedures for the testing of long-term hemocompatibility of coating surfaces with defined geometries subjected to heart-flow conditions. 5) Provide a rapid and reproducible valve fabrication process for industrial uses.
Impacts:
1) Health impact - Benefit for the patients (improved prostethic valve durability, reduced complications and need for revision surgery known to be at very high risk). 2) Health care economic impact - Reduce the complications of prosthetic heart valves and the related health care cost burden. 3) Manufacturing Reproducibility at potentially industrial level (no animal material needed for valve production). 4) Medical Applications: extension of the current technology to percutaneous implantation (much less invasive approach as compared to conventional surgery). 5) Patent/Licence/Spin-off – Job creation/employment opportunities/regional visibilities and know-how.
Combining the outstanding knowledge of the present consortium can guarantee and successfully develop innovative durable polymeric heart valves. We will create a spin-off devoted to the Polymeric Valve to serve as the major channel of valorisation and dissemination of the Poly-Valve concept and outcomes.
Read more Description (FR): Les prothèses valvulaires cardiaques sont le seul traitement efficace chez les patients ayant une valvulopathie symptomatique. Le nombre total de remplacements valvulaires dans les pays de l´OCDE devrait dépasser les 850 000 par an à l’horizon 2050. Les prothèses valvulaires cardiaques actuelles, bien qu’efficaces sur le plan hémodynamique, restent loin d´être optimales en raison de leur détérioration structurelle progressive. De plus, la charge de l´anticoagulation chronique pour les valves mécaniques a des contraintes majeures sur la qualité de la vie. Les problèmes liés à ces valves nécessitent une ré-intervention ou causent le décès d’environ 50% à 60% des patients endéans les 10 ans après implantation d´une première valve prothétique. La prothèse valvulaire idéale ne possèderait pas toutes ces limitations. Les prothèses fabriquées à partir de polymères avancés ont des potentiels de durabilité et d’hémocompatibilité plus élevés. Malheureusement, l’utilisation des valves cardiaques polymériques a été entravée par une tendance à la calcification in vivo conduisant à leur dégradation et par des phénomènes de thrombose délétères. Le présent programme de recherche vise à développer un polyuréthane non toxique dotés de propriétés anti-thrombotique, anti-inflammatoire et anti-calcifiante pour assurer une performance à long terme des valves cardiaques polymériques. D’autre part, en utilisant la technologie d´impression 3D, il sera possible de reproduire avec précision la géométrie des feuillets valvulaires cardiaques, permettant une approche hautement personnalisée des patients souffrant de cette maladie. La production à faible coût de matériaux implantables non toxiques hautement fonctionnels et biocompatibiles pourrait également s’avérer très utile pour diverses autres applications biomédicales.
Objectifs du projet:
• Fournir des polymères de polyuréthane modifiés pour un revêtement des valves cardiaques, et possédant d’excellentes propriétés mécaniques, et de plus grandes durabilité et biocompatibilité.
• Fournir une approche individualisée, basée sur un prototypage 3D, aux patients ayant une maladie valvulaire cardiaque chez qui un remplacement valvulaire chirurgical ou par voie percutanée est planifié.
• Fabriquer un modèle géométrique de haute précision anatomique des feuillets valvulaires correspondant aux dimensions de valves cardiaques grâce à l´utilisation de modèles valvulaires tridimensionnels et de la technologie d´impression 3D.
• Fournir des procédures standardisées pour les essais d’hémocompatibilité à long terme de revêtements, de surfaces définies, soumis à des conditions de flux pulsatile.
Read more Description (DE): Künstliche Herzklappenprothesen sind die einzige wirkungsvolle Therapie für Patienten mit Herzklappenerkrankungen. Die Zahl der jährlich implantierten Klappenprothesen in OECD-Ländern wird bis zum Jahre 2050 auf 850.000 Klappen pro Jahr geschätzt. Die aktuell verwendeten Herzklappenprothese zeigen zwar gute hämodynamische Eigenschaften, gleichzeitig ist jedoch der natürlich Verschleiß von biologischen Prothesen beziehungsweise die Notwendigkeit einer chronischen Antikoagulationstherapie bei mechanischen Prothesen ein gravierender Nachteil. Bei insgesamt 50 % - 60 % aller Patienten führen Komplikationen innerhalb der ersten 10 Jahre nach der Implantation einer künstlichen Herzklappe zu Revisions-Operationen oder sogar dem Tod des Patienten. Daher besteht eine große Notwendigkeit in der Verbesserung künstlicher Herzklappenprothesen, um solche Komplikationen in Zukunft zu vermeiden. Als Ansatz dafür dienen hochentwickelte Polymere, die eine große Dauerfestigkeit sowie eine gute Hämokompatibilität vorweisen. Bislang konnten Polymerherzklappen jedoch noch nicht in der Klinik eingeführt werden, da Degenerierung, Kalzifizierung und Thrombenbildung in-vivo zu Problemen führten.
Im Rahmen des hier beantragen Forschungsvorhabens wird daher eine neues Polyurethan mit verbesserten Thrombose- und Entzündungseigenschaften sowie einer großen Langzeitstabilität entwickelt. Dieses wird mit der automatisierten 3D-Drucktechnologie zu Herzklappenprothesen verarbeitet. Dadurch wird eine hochpräzise Nachbildung der patientenspezifischen nativen Klappengeometrie ermöglicht, sodass ein optimal angepasster Herzklappenersatz maßgeschneidert für den Patienten hergestellt werden kann. Darüber hinaus kommt die Entwicklung des kostengünstigen und nicht-toxischen Polyurethans bei gleichzeitig exzellenter Biokompatibilität und mechanischer Eigenschaften auch anderen Bereichen der biomedizinischen Anwendung zu Gute.

Ac.biomed GmbH (Aachen) offers comprehensive accelerated wear tests (ISO 5840, FDA) for the development and regulatory approval of heart valve prostheses.
