L’accompagnement des patients à leur sortie d’hospitalisation est différemment géré de part et d’autre de la frontière franco-belge. Avec le développement des soins transfrontaliers, au cours de ces dernières années, cette différence s’est accentuée. Si une coordination des soins et des services existe en Belgique, elle reste perfectible : les solutions sont parfois difficiles à mettre en œuvre pour certains patients, notamment en zone rurale. Côté français, l’offre permettant le maintien à domicile après une hospitalisation n’est pas connue des établissements de santé belges du fait de l’absence de caractère global de l’aide accordée au patient selon son statut (personnes âgées, handicapées, …). En conséquence, les patients français ne bénéficient pas des mêmes prestations ni du même accompagnement que les patients belges. Le projet vise alors trois défis importants: - améliorer la sortie d’hôpital et le retour au domicile, tant au niveau de l’aménagement du domicile, la mise en place d’aides et de services (repas, livraisons diverses) qu’au niveau financier. - mettre en place une structuration globale des services favorisant la prise en charge à domicile des patients. - augmenter l’offre de prestataires de soins à domicile en facilitant la mobilité transfrontalière de ceux-ci. L’objectif de cette coopération est de réduire l’enclavement social de la population vivant dans les zones transfrontalières en favorisant les démarches dans les domaines sanitaire et social.

The support provided to patients after discharge from hospital is managed differently on both sides of the Franco-Belgian border. In recent years, this difference has been accentuated with the development of cross-border care. Even though there is an existing coordination of care and services in Belgium, there is room for improvement: solutions are sometimes difficult to implement for some patients, particularly in rural areas. On the French side, the offer enabling home care after hospitalisation is not known to Belgian health facilities due to the lack of comprehensiveness of the assistance granted to the patient according to their status (elderly, disabled, etc.) As a consequence, French patients do not benefit from the same services or the same support as Belgian patients. The project therefore takes up three important challenges: - improve the discharge from hospital and the return home, both in terms of adjustments to the home, the organisation of assistance and services (meals, various deliveries) and at a financial level. - set up an overall structuring of services promoting home care of patients. - increase the supply of home care providers by facilitating their cross-border mobility. This cooperation aims to reduce the social isolation of the population living in cross-border areas by promoting health and social initiatives.

De steun aan patiënten nadat ze uit het ziekenhuis worden ontslagen, wordt aan de twee zijden van de Frans-Belgische grens op verschillende manieren aangepakt. De jongste jaren kwamen deze verschillen nog sterker tot uiting door de ontwikkeling van grensoverschrijdende gezondheidszorg. Hoewel de zorgverstrekking en de diensten in België worden gecoördineerd, is er nog veel ruimte voor verbetering. Sommige patiënten, vooral in landelijke zones, hebben soms moeilijk toegang tot de oplossingen. De Belgische medische instellingen zijn niet op de hoogte van het aanbod van thuiszorg na een ziekenhuisopname aan de Franse zijde, omdat er geen globaal kader bestaat voor de hulp aan de patiënten afhankelijk van hun status (bejaarde, persoon met een beperking enz.). Het gevolg is dat Franse patiënten niet dezelfde diensten of steun ontvangen als Belgische patiënten. Daarom streeft het project drie grote doelstellingen na: - verbetering van het ontslag uit het ziekenhuis en de terugkeer naar huis in termen van inrichting van de woning, organisatie van de bijstand en de diensten (maaltijden, levering aan huis) en financiële aspecten; - globale structurering van de diensten die de thuiszorg van de patiënten bevorderen; - uitbreiding van het aanbod van thuiszorgverleners door hun grensoverschrijdende mobiliteit te verbeteren. Deze samenwerking beoogt het sociale isolement van de bevolking in grensgebieden te verminderen door de promotie van gezondheids- en sociale initiatieven.

COSERDO est un projet pilote de coordination des soins et des services à domicile mené à l'initiative de la Centrale de Services à Domicile de la Province de Namur, de la mutualité Solidaris de la Province de Namur, de l'Union Nationale des Mutualités Socialistes et de la Mutualité Générale de l'Education Nationale grâce au soutien des fonds INTERREG V. Ce projet consiste à mettre en place des outils à destination des hôpitaux et des services de coordination accueillant des patients de l'autre versant de la frontière (cadastre, avantages sociaux existants, accès à la profession, ) : une analyse des métiers «soins et services d'aide à domicile» et leur comparaison, une coordination transfrontalière en matière de soins et d'aide à domicile, une structure de maintenance de la coordination et des procédures établies. Le rôle de la coordination est d’informer, conseiller et aider la mise en place des services nécessaires et les plus appropriés au maintien à domicile, dans le respect du libre choix du patient. La coordination a un également un rôle de facilitateur dans les démarches du bénéficiaire et de son entourage en leur évitant de nombreuses recherches, trouvant ainsi le service adéquat qui répond le mieux aux besoins de la personne. Elle sera le soutien de la famille et les aidants proches en cherchant des solutions concrètes et en facilitant la communication entre tous les intervenants du domicile pour être relais des informations vers le bénéficiaire et les prestataires. La coordination doit être une structuration globale des services favorisant la prise en charge à domicile des patients afin d’améliorer la sortie d’hôpital et le retour au domicile, tant au niveau de l’aménagement du domicile, des services offerts (repas, organisations des soins, …) qu’au niveau financier ; et augmenter l’offre de prestataires de soins à domicile en favorisant la mobilité. Cette coordination a été accueillie favorablement par les divers dispositifs existant sur les 2 versants. Les coordinatrices se sont insérées dans le maillage des structures régionales du territoire français et ont développé des collaborations et synergies avec divers organismes tels que le Conseil départemental, l'ASMUP 08, Arduinn’âge, les Maisons de la solidarité, le MAIA, Assystel, ... Elles ont investi une place laissée vacante par les systèmes français très sectorialisés soit par type de soins, soit par âge (avant 65 et après 65 ans), soit par la situation (personnes handicapées). La mise en place de COSERDO a permis aux établissements hospitaliers belges et français d'obtenir une réponse rapide et efficace aux retours des patients hospitalisés. De ces constats, la coordination est sollicitée aussi bien par les établissements, par les familles, par les acteurs de terrain que par les dispositifs existants. Les demandes administratives et de prises en charge de cesse de s'accroître. En termes d'activité, COSERDO a donné lieu à 367 prises en charge du 1er septembre 2017 au 31 mars 2020. Afin de travailler en toute sécurité, des conventions de collaboration sont signées entre la coordination et les acteurs du maintien à domicile. Dans le cadre de la mobilité des prestataires, le projet avait comme objectif de réaliser des fiches comparatives des métiers à domicile, de réaliser un rapport synthétisant les métiers, et les services à domicile sur les deux versants. Six fiches comparatives ont été réalisées et portent sur les métiers suivants : infirmier, médecin généraliste, kinésithérapeute, ergothérapeute, aide-soignant et une fiche globale « métiers d'aide à la personne à domicile » qui reprend les métiers d'accompagnement éducatif et social (AES), aide familiale, garde malade et aide-ménagère. Les fiches infirmier, médecin généraliste et kinésithérapeute ont été distribuées à tous les prestataires des Zoast Ardennes et Thiérache et pour les autres fiches, où l'identification des destinataires n'étaient pas possibles, ont été envoyées à toutes les écoles formant à la profession de la zone et plus largement si inexistants sur le territoire. Cela représente pour le métier d’aide-soignant (zone géographique du projet) : quatre écoles belges et une française, pour le métier d’ergothérapeute (Wallonie et Départements) : deux écoles belges et aucune française et enfin pour les métiers « d'aide à la personne à domicile » (zone géographique du projet): vingt-deux écoles belges et une française. En France 125 fiches infirmiers , 61 médecins et 78 pour les kinésithérapeutes ont été envoyés. L'ensemble des fiches et leur éventuelle mise à jour sont disponibles sous forme PDF sur le site du projet. Le rapport synthétisant les métiers et services à domicile aborde le thème de l'aide et du maintien à domicile par le biais des chapitres suivants : les services existants de part et d'autre de la frontière, une description des métiers pratiquant à domicile, la présentation de la coordination transfrontalière et ainsi que la présentation des procédures d'installation possibles des prestataires que ce soit pour un établissement permanent sur l'autre versant, l'installation sur les deux versants simultanément ou dans le cadre de l'exercice temporaire ou occasionnel. COSERDO is a pilot project for the coordination of home care and services led by the Home Services Central of the Province of Namur, the Solidaris mutuality of the Province of Namur, the National Union of Socialist Mutualities and the Mutualité Générale de l’Education Nationale thanks to the support of INTERREG V funds. This project consists of setting up tools for hospitals and coordination services for patients from the other side of the border (cadastre, existing social benefits, access to the profession,): an analysis of the “home care and services” trades and their comparison, cross-border coordination in care and home care, a structure for maintaining coordination and established procedures. The role of coordination is to inform, advise and assist in the establishment of the necessary and most appropriate services for keeping at home, while respecting the free choice of the patient. Coordination also plays a facilitating role in the approaches of the beneficiary and his entourage by avoiding numerous researches, thus finding the right service that best meets the needs of the person. It will be the support of the family and close caregivers by seeking concrete solutions and facilitating communication between all those involved in the home to relay information to the beneficiary and the providers. Coordination must be a comprehensive structuring of services favouring the home care of patients in order to improve hospital discharge and return to home, both in terms of home layout, services offered (meals, care organisations, etc.) and at financial level; and increase the supply of home care providers by promoting mobility. This coordination was welcomed by the various mechanisms on both sides. The coordinators have become part of the network of regional structures of the French territory and have developed collaborations and synergies with various bodies such as the Departmental Council, ASMUP 08, Arduinn’age, Maisons de la solidarité, MAIA, Assystel, etc. They have invested a place left vacant by the very sectoral French systems either by type of care, by age (before 65 and after 65 years) or by the situation (persons with disabilities). The establishment of COSERDO has enabled the Belgian and French hospitals to obtain a quick and effective response to the returns of hospitalised patients. From these findings, coordination is sought both by institutions, by families, by actors in the field as well as by existing mechanisms. Administrative and support requests continue to increase. In terms of activity, COSERDO gave rise to 367 takeovers from 1 September 2017 to 31 March 2020. In order to work safely, collaboration agreements are signed between the coordination and the actors of homekeeping. As part of the mobility of providers, the objective of the project was to make comparative fact sheets of the jobs at home, to produce a report summarising the trades, and the home services on both sides. Six comparative sheets have been drawn up and cover the following occupations: nurse, general practitioner, physiotherapist, occupational therapist, caregiver and a global “home care profession” which takes over the professions of educational and social accompaniment (AES), family help, sick care and home help. The nurses, general practitioners and physiotherapists were distributed to all the providers of the ZOAST Ardennes and Thiérache and for the other forms, where the identification of the recipients were not possible, were sent to all the schools training in the profession of the area and more broadly if non-existent in the territory. This means for the caretaker’s job (geographical area of the project): four Belgian schools and one French, for the profession of occupational therapist (Wallonie and Departments): two Belgian schools and no French schools and finally for “home help” trades (geographical area of the project): twenty-two Belgian schools and one French. In France 125 nursing records, 61 doctors and 78 for physiotherapists were sent. All the fiches and their possible updating are available in PDF form on the project website. The report synthesising home trades and services addresses the theme of home support and retention through the following chapters: existing services on both sides of the border, a description of the occupations working at home, the presentation of cross-border coordination and the presentation of the possible installation procedures of the providers, whether for one permanent establishment on the other side, the installation on both sides simultaneously or as part of the temporary or occasional exercise. Nombre de personnes ayant utilisé un service en matière sanitaire de part et d'autre de la frontière : 10 000. Il s’agit d’une estimation sur la base du nb de personnes ayant bénéficié de la coordination belge, soit 570/an, et sur le nb de patients français hospitalisés sur la zone en 2014, soit 2 000 hospitalisations annuelles. Par conséquent, un potentiel annuel de 2 570 personnes est estimé, soit un total de 10 280 personnes.

Expected Results (EN):

Number of people who have used a health service on both sides of the border: 10,000. It consists of an estimate on the basis of the number of persons having benefited from the Belgian coordination, that is 570/year, and on the number of French patients hospitalised across the region in 2014, thus 2,000 annual hospitalisations. Consequently, an annual potential of 2,570 people is estimated, for a total of 10,280 people.

Aantal mensen die hebben gebruikgemaakt van een gezondheidsdienst aan beide zijden van de grens: 10.000. Dit is een raming op basis van het aantal personen die hebben gebruikgemaakt van de Belgische coördinatie, namelijk 570/jaar, en het aantal Franse patiënten die in de volledige regio werden gehospitaliseerd in 2014, namelijk 2000 hospitalisaties per jaar. Bijgevolg wordt een jaarlijks potentieel van 2570 mensen geschat voor een totaal van 10.280 mensen.

Nombre de dispositifs en matière d’accès aux services sanitaires et sociaux de part et d’autre de la frontière : 13. Les dispositifs suivants seront réalisés : la mise en réseau d’un service de coordination transfrontalier, le rapport d’équivalence des métiers, la fiche pratique à l’égard des patients, les fiches pratiques à l’égard des professionnels de la santé.

The following schemes will be implemented: the networking of a cross-border coordination service, the equivalence ratio of professions, the practical information sheet for patients, the practical information sheets for health professionals.
