Description (EN): The methods of treating lung patients face changes. Biological medicines can treat lung patients in new ways, but to optimise their use, biomarkers are needed. The BREATHE project will solve this.
For patients with COPD and asthma, new biological products are available on the market. Unlike current inhalation treatments, the biological medications instead treat the cause of the lung diseases. But the biological medications are more expensive and treat a narrower patient group. Therefore, these groups need to be studied.
The challenge is that the biological medications do not work equally well on all patients. Asthma is not just asthma, but there are different types of asthma. The same applies to COPD. With the biological products, we have one product that works on asthma type a, and another that works on asthma type b. The challenge is to see which patients should have which products, says project manager Thomas Veje Flintegaard.
Personalise the medication:
In addition, the products are up to 10 times as expensive as the current lung medications, and therefore it is especially important that patients are treated with the right kind of biological medicine.
In the Öresund area, we have about 500,000 asthma and COPD patients and we believe that up to 30 % of them can use the biological products, it can be quite a big expense if they are all to have the biological medicines. But we really want to give our patients the best treatments possible. Therefore, in order to be able to use the biological medicines in the best way, biomarkers are needed to show who should have what medication, he explains.
Biomarkers can, for example, be proteins that are expressed in a particular way for different types of asthma diseases. By identifying biomarkers, you can personalise the medication.
We will also investigate how we can implement E-Health in the treatment of lung diseases, as we believe that this, together with better medication, can significantly improve the lives of people with asthma or carbon.
The collaboration broadens the patient base:
The treatment of lung patients differs between Sweden and Denmark. In Denmark, most patients come directly to hospitals, while in Sweden they have pulmonary specialists in primary care. By studying patients in both countries, the project gets a broader patient base.
In this way, we have a good patient group to start from. It creates a breadth and we can see how the products work on a “real life” patient base.
A total of 1800 asthma and COPD patients should be studied. The project will use common diagnostic methods and take different samples from, among other things, lungs, blood and spit. Here too, cross-border cooperation produces results, as there are different competencies in lung pathology and microbiology among the participants in the project.
The project would not be possible if we did not cooperate. First, we would find it difficult to recruit patients and get the same breadth among them. It also makes no sense not to cooperate when some of the world’s top researchers in the field are located in both countries.
Creating better medication:
Once the project is completed, it is hoped to have such a wide base of biomarkers that you can start medicating biological products in a resource-efficient way. In addition, the project is working on developing a system for patients to be able to treat themselves at home through an app.

För patienter med KOL och astma finns nya, biologiska produkter på marknaden. Till skillnad från nuvarande inhalationsbehandlingar, behandlar de biologiska medicinerna istället orsaken till lungsjukdomarna. Men de biologiska medicinerna är dyrare och behandlar en smalare patientgrupp. Därför behöver dessa grupper studeras.
– Utmaningen är att de biologiska medicinerna inte verkar lika gott på alla patienter. Astma är nämligen inte bara astma, utan det finns olika sorters astma. Desamma gäller för KOL. Med de biologiska produkterna så har vi en produkt som verkar på astmatyp a, och en annan som verkar på astmatyp b. Utmaningen är att se vilka patienter som ska ha vilka produkter, säger projektledare Thomas Veje Flintegaard.
Personifiera medicineringen:
Produkterna är dessutom uppemot 10 gånger så dyra som de nuvarande lungmedicinerna, och därför är det särskilt viktigt att patienter behandlas med rätt sorts biologisk medicin.
– I Öresundsområdet har vi omkring 500 000 astma- och KOL patienter och vi tror att upp emot 30 procent av dem kan använda de biologiska produkterna, det kan bli en rätt så stor utgift om de alla ska ha de biologiska medicinerna. Men vi vill riktigt gärna ge våra patienter så bra behandlingar som möjligt. För att kunna använda de biologiska medicinerna på bästa sätt behövs därför biomarkörer som visar vem som ska ha vilken medicin, förklarar han.
Biomarkörer kan exempelvis vara proteiner som uttrycks på ett särskilt sätt för olika typer av astmasjukdomar. Genom att identifiera biomarkörer kan man personifiera medicineringen.
– Vi ska också undersöka hur vi kan implementera E-Health i behandlingen av lungsjukdomar, då vi tror att detta tillsammans med bättre medicinering, kan förbättra livet för personer med astma eller kol markant.
Samarbetet breddar patientunderlaget:
Hanteringen av lungpatienter skiljer sig åt mellan Sverige och Danmark. I Danmark kommer de flesta patienterna direkt till sjukhusen, medan man i Sverige har lungspecialister i primärvården. Genom att studera patienter i båda länderna får projektet ett bredare patientunderlag.
– På så sätt får vi en bra patientgrupp att utgå ifrån. Det skapar en bredd och vi kan se hur produkterna fungerar på ett ”real life” patientunderlag.
Totalt ska 1800 astma- och KOL-patienter studeras. Projektet kommer att använda vanliga diagnostiska metod och ta olika prover från bland annat lungor, blod och spott. Även här ger det gränsöverskridande samarbetet resultat, eftersom det finns olika kompetenser inom lungpatologi och mikrobiologi bland projektets deltagare.
– Projektet skulle inte vara möjligt om vi inte samarbetade. För det första skulle vi ha svårt att rekrytera patienter och få in samma bredd bland dem. Det ger heller inte någon mening att inte samarbeta när några av världens främsta forskare inom området finns i de båda länderna.
Skapar bättre medicinering:
När projektet är avslutat hoppas man ha ett så pass brett underlag av biomarkörer att man kan börja medicinera biologiska produkter på ett resurseffektivt sätt. Dessutom arbetar projektet med att ta fram ett system för patienter att kunna behandla sig hemma genom en app.
