The demands on a health system are very knowledge-intensive due to their complexity. This cannot be solved alone nor regionally; only through target-oriented and action-oriented cooperation. The COOP4HEALTHCARE project aims, through its main objective, to improve the provision of services in the health sector through consistent cross-border cooperation between the relevant actors in the program area and with strategic European partners. Through this, highly-efficient, focused and long-term oriented pilot project (digitalisation, knowledge excellence, & cooperation models) are activated and implemented. Therefore, it addresses the challenges of improving health cooperation and providing balanced, modern, and high-quality care to all population areas on a local and independent basis. The planned change is primarily generated through integrated health solutions; in particular through the main outputs (1) knowledge and cooperation-oriented work base; (2) excellent pilot projects; (3) action-oriented roadmap 2020-2026 and the (4) HEALTHCARE Think Tank. COOP4HEALTHCARE fosters proven tools and methodologies that are further developed and adapted for use in integrated health solutions. The new approach to the project is the structured use of operational and strategic instruments, as well as their consistent, permanent, and organizational anchoring. Due to the high degree of implementation (qualities and quantities) during the project and beyond the project’s duration, a significant benefit is achieved for the target groups: (1) 5,600 people profit directly from the improved and new services and (2) 42 projects are developed, some directly implemented and numerous other project activities are anchored in multi-year, long term action plans.

Die Anforderungen an ein Gesundheitssystem sind aufgrund der Komplexität sehr wissensintensiv. Dies kann per se nicht allein, auch nicht regional, sondern in ziel- und aktionsorientierten Kooperationen gelöst werden. Das Projekt COOP4HEALTHCARE forciert im übergeordneten Projektziel eine Verbesserung des Dienstleistungsangebots im Gesundheitsbereich durch eine konsequente grenzüberschreitende Kooperation der relevanten Akteure im Programmgebiet und mit strategischen Europäischen Partnern. Damit werden direkte und langfristig orientierte Pilotprojekte mit einem hohen Wirkungsgrad (Digitalisierung, Wissensexzellenz, Kooperationsmodelle) aktiviert und umgesetzt. Somit wird auch gezielt den Herausforderungen, die Zusammenarbeit im Bereich Gesundheit zu verbessern und ein eine ausgeglichene, qualitativ hochwertige Versorgung allen Bevölkerungsgruppen örtlich unabhängig und zeitgerecht zur Verfügung zu stellen, begegnet. Die geplante Veränderung erfolgt hauptsächlich durch integrierte Gesundheitslösungen, die insbesondere die von den Hauptoutputs (1) Arbeitsbasis: Wissen & Lernen; (2) Pilotprojekte mit Exzellenzcharakter; (3) aktionsorientierte Roadmap 2020-2026 und (4) HEALTHCARE Think-Tank geschaffen werden. COOP4HEALTHCARE forciert erprobte Tools und Methodiken, die für den Einsatz von integrierten Gesundheitslösungen weiterentwickelt und adaptiert werden. Das Neue am Projektansatz ist der strukturierte Einsatz von operativen und strategischen Instrumenten sowie deren konsequente dauerhafte, auch organisatorische Verankerung. Durch den hohen Umsetzungsgrad (Qualitäten und Quantitäten) im Projekt und über die Projektlaufzeit hinaus wird ein signifikanter Nutzen für die Zielgruppen erzielt: (1) 5.600 Personen profitieren direkt von den verbesserten und neuen Dienstleistungen; (2) 42 Projekte teils direkt umgesetzt und zahlreiche weitere Projekte in mehrjährigen Aktionsplänen verankert.

Zahteve za zdravstveni sistem zaradi kompleksnosti v precejšnji meri temeljijo na znanju. Tega ni mogoče reševati samostojno, niti ne na regionalni ravni, temveč s ciljno in akcijsko usmerjenim sodelovanjem. Skupni glavni cilj projekta COOP4HEALTHCARE je izboljšanje ponudbe storitev na področju zdravstva na podlagi konsekventnega čezmejnega sodelovanja relevantnih akterjev s programskega območja in sodelovanja s strateškimi evropskimi partnerji. S tem bodo aktivirani in izvedeni direktni in dolgoročno naravnani pilotni projekti z visoko stopnjo učinkovanja (digitalizacija, odličnost znanja, modeli sodelovanja). Tako se bo projekt usmerjeno soočil tudi z izzivi, kako izboljšati sodelovanje na področju zdravstva in kako vsem skupinam prebivalstva zagotoviti uravnoteženo, visoko kakovostno oskrbo, ki bo pravočasna in ne bo odvisna od lokacije. Do načrtovanih sprememb bo prišlo predvsem s pomočjo integriranih zdravstvenih rešitev, ki jih bodo ustvarili zlasti glavni neposredni učinki: (1) delovna osnova: znanje in učenje; (2) pilotni projekti z značajem odličnosti; (3) akcijsko usmerjen načrt za obdobje 2020–2026; (4) možganski trust HEALTHCARE. Projekt COOP4HEALTHCARE pospešuje preizkušena orodja in metodologije, ki bodo za uporabo integriranih zdravstvenih rešitev še dodatno razvita in prilagojena. Novost pri projektnem pristopu je tako strukturirana uporaba operativnih in strateških instrumentov ter njihova dosledna trajna in tudi organizacijska umestitev. Visoka stopnja izvajanja (s kakovostnega in količinskega vidika) v okviru projekta in po koncu trajanja projekta bo bistveno koristila ciljnim skupinam: (1) 5.600 oseb bo imelo neposredno korist od izboljšanih in novih storitev; (2) 42 projektov bo deloma neposredno izvedenih in mnogi nadaljnji projekti bodo umeščeni v večletnih akcijskih načrtih.

Expected Results (SL): Stopnja trajnostnega institucionalnega sodelovanja na področju energije in obvladovanja tveganj ali zdravja in socialne kohezije v programskem območju

Expected Outputs (DE):

Die Hauptresultate sind eine (1) enge Kooperation von Institutionen für integrierte Lösungen im Aufgabenfeld Gesundheit, die sich auf die 7 Projektpartner und min. weitere 61 Organisationen (siehe Zielgruppen) im Kooperationsraum bezieht sowie über die Projektlaufzeit hinausgeht; (2) das gezielte Entwickeln und Umsetzen von 42 grenzüberschreitenden Pilotprojekten für integrierte Gesundheitslösungen und 1 gemeinsame strategischen Roadmap 2020-2026 und zahlreichen Folgeaktionen; und dadurch (3) eine verbessertes Dienstleistungsangebot für die Bevölkerung im Programmgebiet. Die Rahmenbedingungen dazu werden durch die SIAT Arbeits- & Wissensbasis, die aktionsorientierte Roadmap und den SIAT HEALTHCARE Think-Tank geschaffen, die als dauerhaft nutzbare Resultate verbleiben. Somit werden Kooperationen und die angebotenen Dienstleistungen durch Wissensexzellenz inklusive Digitalisierung permanent gemeinsam mit den Entscheidungsträgern (Policy Learning Lab) forciert und optimiert.

Expected Outputs (SL):

Glavni rezultati so (1) tesno sodelovanje med institucijami za integrirane rešitve na področju dejavnosti zdravstva, ki se nanaša na 7 projektnih partnerjev in najmanj nadaljnjih 61 organizacij (glej ciljne skupine) na območju sodelovanja in presega obdobje trajanja projekta; (2) ciljno usmerjen razvoj in izvajanje 42 čezmejnih pilotnih projektov za integrirane zdravstvene rešitve ter 1 skupen strateški načrt za obdobje 2020-2026 in številne nadaljnje aktivnosti; in s tem (3) izboljšanje ponudbe storitev za prebivalstvo na programskem območju. Okvirni pogoji za to bodo ustvarjeni s slovensko-avstrijsko bazo dela in znanja, akcijsko usmerjenim načrtom in možganskim trustom SIAT HEALTHCARE, ki bodo ostali kot dolgoročno uporabni rezultati. Sodelovanje in ponujene storitve se bodo stalno pospeševale in optimizirale prek odličnosti znanja, vključno z digitalizacijo, v sodelovanju z nosilci odločanja (Policy Learning Lab).
