DiagNet – “Diagnostics of the future at Lake Constance.” Network to develop and apply new diagnostic procedures for on-site application in rural areas.

Das Projekt „DiagNET- Diagnostik der Zukunft“ zielt auf die Entwicklung und Anwendung neuer Diagnoseverfahren in einem Kompetenznetz von Wirtschaft und Wissenschaft in der Vierländerregion Bodensee und darüber hinaus ab. Diese sollen in den Kliniken aber auch in Arztpraxen sowie Laboren vor Ort einsetzbar sein und somit lange Wege ersparen. Damit wird die medizinische Versorgung ländlicher Räume deutlich verbessert. Koordiniert wird das Projekt „DiagNET“ vom BioLAGO e.V. , dem grenzüberschreitenden Gesundheitsnetzwerk in der Vierländerregion Bodensee. Für viele Erkrankungen bzw. Infektionen fehlen Tests, um zu einer sofortigen, gezielten Therapie zu gelangen. Die aufwendigen Tests werden bisher nur in speziellen Zentren (häufig Universitätskliniken) durchgeführt. Für etliche Krankheiten fehlen schnelle und zuverlässige Testverfahren gänzlich. Zudem fehlen häufig vor Ort einsetzbare Testvarianten. Für die Patienten ist die Durchführung dieser diagnostischen Tests somit mit einem hohen Aufwand und Kosten für die Terminabstimmung und Anreise verbunden. Die anschließende Therapie erfolgt meist wieder beim (Haus-) Arzt vor Ort und somit zeitlich verzögert. Bei vielen Erkrankungen kann es jedoch lebensrettend sein, eine schnelle Diagnose und einen auf den Patienten abgestimmten Therapieplan zu erhalten. Z. B. bei multiresistenten Erregern oder Krebserkrankungen. Bei bakteriellen Infektionen werden Antibiotika oft nicht gezielt und damit wenig effizient eingesetzt. Erschwerend kommt hinzu, dass immer mehr antibiotikaresistente Keime auftreten. Dies betrifft nicht nur die Humanmedizin, sondern auch die Veterinärmedizin, wo es massenhaft zu fehlerhaften Antibiotika-Gaben kommt. Bei Krebserkrankungen ist die Frühdiagnose für die Therapierbarkeit von entscheidender Bedeutung. Deshalb sollten neue Testverfahren für eine Vor-Ort-Diagnostik beim behandelnden Arzt entwickelt werden. Diese Entwicklungen erfolgen bisher meist als Einzelaktivität der regionalen Akteure, die nicht durch ein Gesamtkonzept koordiniert werden. Auch auf der Anwenderseite tauschen Kliniken und Klinikverbünde sowie Arztpraxen ihre Erfahrungen noch nicht aus. Das Projekt „DiagNet“ strebt eine gezielte Koordination sowie eine gemeinsame Entwicklung und Anwendung neuer Testverfahren durch Unternehmen, Labore, Hochschulen und Forschungseinrichtungen an. Die Projektthemen für neue Diagnostik-Verfahren im Rahmen des DiagNet-Projekts sind: Schnelltest zur Differenzierung einer bakteriellen Erkrankung in Abgrenzung zu einer Erkrankung, die durch Viren, Pilze etc. verursacht wird (und damit Möglichkeit einer gezielten Therapie bzw. eines gezielten Antibiotika-Einsatzes). Bestimmung zirkulierender Tumorzellen im Blut von Tumorpatienten (z.B. Dickdarmpatienten). Nachweis von multiresistenten Erregern (z. B. MRSA) (z.B. als Schnelltest bei Einlieferung von Patienten). Die Projektphasen und Aufgaben gliedern sich folgendermaßen: Aufbau eines Kompetenznetzes „Diagnostik“ aus Unternehmen, Forschungseinrichtungen sowie Kliniken und Laboren. Entwicklung neuer Diagnoseverfahren und Optimierung bestehender Tests. Etablierung des Kompetenznetzes und Erprobung von Diagnoseverfahren für die Anwendung.   Das Projekt fördert den intensiven Austausch zwischen Unternehmen, Wissenschaft und medizinischer Versorgung zur gemeinsamem Entwicklung neuer Verfahren und verbessert damit die Gesundheitsversorgung besonders in ländlichen Regionen. Es kommt zu einer zügigeren und effektiveren Therapie. Das Projekt „DiagNet“ führt die Forschungs-, Entwicklungs-, Produktions- und Anwenderkompetenz in einem Netzwerk zusammen und fördert damit die Entwicklung und Anwendung neuer Diagnostik-Verfahren für die Menschen vor Ort. Die Ressourcen von Wirtschaft, Wissenschaft und medizinischer Versorgung werden gebündelt. Der Entwicklungsprozess wird beschleunigt. Es sind Partner aus drei Ländern beteiligt, so dass auch drei verschiedene Gesundheitssysteme Berücksichtigung finden werden. Am Projekt nehmen insgesamt 13 Partner teil, davon stammen 7 aus Deutschland, 2 aus Österreich und 4 aus der Schweiz. Krankenhäuser sind im Landkreis Konstanz, in Feldkirch, in Münsterlingen und in St. Gallen eingebunden. Weitere Partner können später hinzukommen. Das Projekt ist ein dynamischer Prozess. Auch nach Projektende arbeitet das Kompetenznetz weiter.

The “DiagNET diagnostics of the future” project aims to develop and apply new diagnostic procedures in a skill network involving science and business, in the four countries of the Lake Constance region and beyond. These are to be deployed in clinics and doctors’ practices as well as laboratories in the area, cutting down long-distance travel. Medical care will thereby be considerably improved in rural areas. The “DiagNET” project is coordinated byBioLAGO e.V., the cross-border health network in the Lake Constance four-country region. Tests, in order to be able to find an immediate targeted therapy, are lacking for many diseases and infections. The time-consuming tests have only been carried out in special centres up to now (often at university clinics). There are no fast and reliable test procedures for a number of diseases. There is frequently also a lack of test variants that can be deployed on-site. For the patient, the conducting of these diagnostic tests therefore entails a great deal of effort and high costs in arranging the appointment and travelling. The associated therapy is usually carried out by the general practitioner at their practice, so there is often a time delay. Receiving a quick diagnosis and a therapy plan adapted to the patient can be life-saving in the case of many diseases. E.g. for multi-resistant pathogens or cancer. Antibiotics are often not targeted and are therefore inefficiently deployed for bacterial infections. This is hindered by the fact that germs are increasingly emerging that are antibiotic-resistant. This not only applies in human medicine but in veterinary medicine too, where incorrect administration of antibiotics occurs on a large scale. Early diagnosis is absolutely crucial for the ability to treat cancer. Therefore new test procedures should be developed for sur place diagnosis by the doctor treating it. These developments mainly occur as an individual activity by regional parties, which are not coordinated by an overall concept. From the user’s perspective too, clinics and clinic groups as well as medical practices do not exchange experiences. The “DiagNet” project pursues targeted coordination as well as the joint development and application of new test procedures by companies, laboratories, universities and research facilities. The project topics for new diagnostic procedures in the context of the DiagNet project are:  A quick test to differentiate a bacterial illness from an illness caused by viruses, fungi, etc. (and thus the option for targeted therapy or the targeted use of antibiotics).  Identification of tumour cells circulating in the blood of tumour patients (e.g. colon patients).  Proof of multi-resistant pathogens (e.g. MRSA) (for example, as a quick test when patients are admitted). The project phases and tasks are divided up as follows: - Setting up of a “diagnostic” skill network consisting of companies, research facilities, clinics and laboratories - Development of new diagnostic procedures and optimisation of existing tests - Establishment of the skill network and testing of the diagnostic procedure for the application - Laboratory 4.0 – intelligent digitalisation and automation for SMEs in the Lake Constance region (funded by: the ILCC 2015-20 small project fund in the Interreg V “Alpine Rhine-Lake Constance-High Rhine" programme) The project promotes intensive exchange between companies, science and medical care for the joint development of new procedures, and thereby improves healthcare, particularly in rural regions. Treatment will be faster and more effective as a result. The “DiagNet” project brings together research, development, production and user skills in a network and thereby encourages the development and application of new diagnostic procedures for people sur place. The resources of business, science and medical care are grouped together. The development process is speeded up. Partners from three countries are involved, so that three different health systems will also be considered. A total of 13 partners are taking part in the project, 7 of whom come from Germany, 2 from Austria and 4 from Switzerland. The hospitals involved are in the Constance district, Feldkirch, Münsterlingen and St. Gallen. Other partners can join later on. The project is a dynamic process. The skill network will continue to work together after the project is completed.

Das Projekt „DiagNET- Diagnostik der Zukunft“ zielt auf die Entwicklung und Anwendung neuer Diagnoseverfahren in einem Kompetenznetz von Wirtschaft, Wissenschaft und Medizin in der Vierländerregion Bodensee und darüber hinaus ab. Im Fokus stehen diagnostische Verfahren zum Nachweis gefährlicher Krankenhauskeime, zur Antibiotika- Kontrolle in der Intensivmedizin, zur Messbarkeit eines gesunden Lebensstils sowie zur Digitalisierung in der Diagnostik. Im Fokus des Projekts stand der Aufbau eines Kompetenznetzes für Diagnostik, bestehend aus Unternehmen, Forschungseinrichtungen und Akteuren der Gesundheitsversorgung. Das Netzwerk deckt die gesamte Wertschöpfungskette ab: von der medizinischen Problemstellung über die technische Umsetzung bis hin zur Implementierung und Einsatz beim Patienten. Ziel war es, länderübergreifend Arbeitsgruppen bzw. Konsortien zu initiieren und zu begleiten, die gemeinsam Konzepte für eine verbesserte Diagnostik entwickeln und deren Umsetzung in (geförderten) Projekten anzustoßen. Dies wurde z.B. mit dem Forschungsprojekt „DiabKON: Diagnostischer Test für die Antibiotikatherapie-Kontrolle in der Intensivmedizin“, dass durch das Bundesforschungsministerium mit 1,8 Mio. Euro gefördert wird, erreicht (weitere Infos unter Absatz „Projektbewertung“). Mit über 50 Partnern, davon etwa 35 Unternehmen aus 4 Bodenseeländern (Deutschland, Schweiz, Österreich, Liechtenstein) sowie weiteren europäischen Staaten (Irland und Spanien) stieß das Projekt auf sehr hohes Interesse bei Fachunternehmen, Ärzten, Laboren und Versorgungseinrichtungen. Insbesondere kleine und mittlere Unternehmen haben sich aktiv am Projekt beteiligt und davon in Form von u.a. Kooperationen untereinander sowie mit der Wissenschaft profitiert. Es wurden 4 Arbeitsgruppen (Multiresistente Erreger, Therapie-Kontrolle, Digitalisierung sowie Lifestyle- Diagnostik) initiiert, in denen eine grenzüberschreitende Interaktion gelebt wurde. In Status- Workshops wurden die Ergebnisse und Erfahrungen der Arbeitsgruppen ausgetauscht und dienten der Quervernetzung sowie der Gewinnung weiterer Partner. Die Nachfrage nach einer gezielten Beratung und Begleitung der Projektteilnehmer durch BioLAGO war stets sehr hoch. Diese bezog sich auf die Suche nach passenden Kooperationspartnern, auf Marktbewertungen sowie Förder- und Finanzierungsmittel. Mit dem DiagNET-Kompetenznetz wurde eine Bedarfslücke geschlossen. Vergleichbare Netzwerke sind lediglich in Jena und Berlin ansässig, jedoch nicht grenzüberschreitend. Ziel des Projekts war es daher auch, die Arbeitsergebnisse über eine aktive Öffentlichkeitsarbeit (Website, Newsletter, Pressebeiträge, Veranstaltungen) bekannt zu machen und den Wissenstransfer voranzutreiben. Das DiagNET konnte als einziges grenzüberschreitendes Diagnostik-Kompetenznetz in D, CH und A in der Branche positioniert werden. Ein Kernelement des Projekts war die Initiierung von zwei internationalen Fachkonferenzen für die Diagnostik. Mit insgesamt über 300 Teilnehmern aus über 10 Ländern hat sich die „Diagnostics-4-Future Conference“ als Treffpunkt der Branche länderübergreifend profiliert.

The project “DiagNET Diagnostics of the Future” aims at the development and application of new diagnostic methods in a competence network of business, science and medicine in the four-country region of Lake Constance and beyond. These should also be used in the clinics but also in doctor’s offices and laboratories on site, thus saving long distances. This significantly improves the medical care of rural areas. The focus is on diagnostic procedures for the detection of dangerous hospital germs, antibiotic control in intensive care, the measurability of a healthy lifestyle and digitalisation in diagnostics. The focus of the project was the development of a competence network for diagnostics, consisting of companies, research institutions and health care actors. The network covers the entire value chain: from the medical problem to the technical implementation to the implementation and use of the patient. The aim was to initiate and accompany cross-country working groups or consortia, who jointly develop concepts for improved diagnostics and initiate their implementation in (funded) projects. This was achieved, for example, with the research project “DiabKON: Diagnostic Test for Antibiotic Therapy Control in Intensive Care”, which is funded by the Federal Ministry of Research with 1.8 million euros (more information under “Project Evaluation”). With more than 50 partners, about 35 companies from 4 countries of Constance (Germany, Switzerland, Austria, Liechtenstein) as well as other European countries (Ireland and Spain), the project attracted very high interest from specialist companies, doctors, laboratories and utilities. In particular, small and medium-sized enterprises have actively participated in the project and benefited from this in the form of, inter alia, cooperations with each other and with science. Four working groups (multi-resistant pathogens, therapy control, digitalisation and lifestyle diagnostics) were initiated in which a cross-border interaction was lived. In status workshops, the results and experiences of the working groups were exchanged and were used to cross-link and attract further partners. The demand for targeted advice and support of the project participants by BioLAGO has always been very high. This related to the search for suitable cooperation partners, market evaluations as well as funding and funding. The DiagNET competence network closed a gap in demand. Comparable networks are located only in Jena and Berlin, but not cross-border. The aim of the project was therefore to promote the results of the work through active public relations (website, newsletters, press articles, events) and to promote knowledge transfer. DiagNET was the only cross-border diagnostic competence network to be positioned in the industry in D, CH and A. A key element of the project was the initiation of two international expert conferences for diagnostics. With a total of more than 300 participants from more than 10 countries, the “Diagnostics-4-Future Conference” has distinguished itself as a meeting place for the industry across countries.

Ziel des DiagNet-Projektes und damit des Netzwerkes ist es, neue Verfahren zu entwickeln. Hierzu ist eine Steigerung der Forschungs- und Entwicklungsausgaben notwendig, diese werden die Projektpartner (im eigenen Interesse) erhöhen (SZ 2). Zudem wird der Ergebnisindikator SZ 8 erfüllt: Schon in der Antragsvorbereitung und Identifikation der Projektthemen kamen Unternehmen und Hochschulen, die bisher nicht zusammengearbeitet bzw. noch keinen Kontakt hatten, zusammen. Mit einer Reduktion der CO2-Emission (SZ 4) wird aufgrund der Herabsetzung der Patientenwege gerechnet.
