Description (EN): The landscape: challenge and added value of cooperation:
Cancer is a growing challenge for society. Its incidence is increasing with aging, and cancer is turning into a chronic disease. Besides classical treatment approaches improvement, many innovative treatments are emerging (immunotherapy, gene-targeted therapies). In addition to longer survival, better quality-of-life and costs are becoming prominent issues.
To face these challenges, academic hospitals and universities are developing comprehensive cancer centers, integrating multidisciplinary care managed in a quality-oriented manner, clinical/translational research and teaching. Focus on the patient and innovation allows to create organized networks, in which cooperation is set forth instead of competition.
Our teams are dedicated to provide optimized care to patients in the Euregio, constantly seeking to improve services. Having innovative ideas, but lacking resources and/or methodology to translate them rapidly into practice, they suggest to exchange their winning solutions and to combine their assets. Our project proposes synergistic strategies to provide sustainable innovation and quality of care, thereby contributing to regional development and well-being of the population. It is based on collaboration between 5 academic institutions and 1 private company. The proof of concept of different tools in small and large non-academic hospitals will allow to outreach the program to rural and urban parts of the Euregio Meuse-Rhin.
Project objectives and activities:
OncoCare has an ambitious vision of improving the quality of cancer care throughout the Euregio. It proposes well-developed and realistic actions focusing on organizational and technological innovation with immediate results for the population. The project is structured in 3 actions (WP 3-4-5) whose challenges are describing in C.1.1 and their solutions in C.1.2.
Axis 1 - Patient-centered medicine (WP3): The 1st action will set-up a teleconsulting network through which scientific & medical experts can provide personalized advice on partner and associated institutions patients. A 2nd action will develop a portable technology allowing research on patient assessment of the real added value of treatments (PROM). The 3rd action will develop a network of health professionals providing specialized cancer care at home.
Axis 2 - Quality of care (WP4): We aim at developing quality management (QM) thanks to accreditation. We will harmonize cancer registries, facilitate data exchange for quality control, benchmarking, and clinical research and we will set-up efficient QM systems in our cancer centers. Furthermore, we will develop software for the global management of hematopoietic stem cell transplantation (HSCT), facilitating research and accreditation.
Axis 3 - Access to innovative treatment (WP5) : We aim at fostering innovation opportunities and transferring them rapidly to the clinic. This action will develop cara pathways and guidelines and will share them with associated institutions. Furthermore, to facilitate access to innovative treatments, we will set-up a common research platform of clinical trials (CT), including a concerted strategy on rare cancer.
Main results & outputs:
Each action delineates expected results (C.1.2) and deliverables from WP 3-4-5. Great care will be devoted to durable establishment of tools (CT platform and care pathways, HSCT management tool, PROM tool, cancer registry, care pathway platform, QM systems) in order that partner institutions benefit from improved quality of their processes, develop research and comply with regulatory requirements via accreditations.
Durability and added value of our collaboration will greatly increase by transferring various tools (home care network, teleconsulting, patient registry, care pathway and CT platforms) to associated institutions as well as by the exchanges with national/regional registries (e.g. LKR in NRW), thus synergistic trans-border network in the fight against cancer.
Patients’ perception of healthcare quality highly depends on the quality of communication with healthcare providers. Facilitation of evidence-based personalized decision-making, patient-reported outcome measurements, access to innovative therapies and HAD will be perceived as great direct benefits. Within WP2 (Communication), our project emphasizes transparent communication on the project’s output to stakeholders (researchers, patient associations, general practitioners, oncology professionals…) and the general public. We also seek feed-back from patients, self-help groups, researchers, health professionals and deciders.

Le cancer est un défi grandissant pour la société. Son incidence augmente avec le vieillissement et il devient une maladie chronique. Outre l';amélioration des traitements conventionnels, des thérapies innovantes (immunothérapie, thérapies ciblées) émergent. Au-delà de la survie, la qualité de vie et les coûts deviennent des enjeux importants.
Pour relever ces défis, les hôpitaux universitaires et les universités développent des centres de lutte contre le cancer, intégrant gestion multidisciplinaire de soins de qualité, recherche clinique/translationnelle et enseignement. Un focus sur le patient et l’innovation nous permettent de constituer de véritables réseaux, dans lesquels la coopération prévaut sur la compétition.
Nos équipes s’engagent à offrir des soins de qualité aux patients de l';Euregio en améliorant continuellement les services. Ne manquant pas d’idées innovantes, mais ne disposant pas de toutes les ressources et méthodologies nécessaires, elles proposent d’échanger leurs solutions gagnantes et de combiner leurs forces respectives. Notre projet propose des stratégies synergiques pour une innovation durable et la qualité des soins, contribuant ainsi au développement régional et au bien-être de la population. Il se base sur une collaboration étroite entre 5 institutions universitaires et une société privée. La preuve de concept des différents outils dans de petits et grands hôpitaux non-universitaires permettra d’étendre notre programme à toutes les régions rurales et urbaines de l';Euregio Meuse-Rhin.
Objectifs et contenu du projet:
Oncocare a une vision ambitieuse de l';amélioration de la qualité des soins oncologiques dans l';ensemble de l';Euregio. Il propose des actions bien élaborées et réalistes mettant l’accent sur l';innovation organisationnelle et technologique avec des résultats immédiats pour la population. Il est structuré en 3 actions (WP 3-4-5) dont les défis sont décrits en C.1.1 et leurs solutions en C.1.2.
Action 1- Médecine centrée sur le patient (WP3). La 1e activité développera un réseau de téléconsultations d’experts scientifiques et médicaux fournissant des conseils personnalisés sur certains patients des institutions partenaires et associées. La 2e développera une technologie portable permettant la recherche sur l’évaluation par les patients de la valeur ajoutée réelle des traitements reçus (PROM). La 3e mettra sur pied l’hospitalisation à domicile.
Action 2 - Qualité des soins (WP4). Notre ambition est de développer une gestion de la qualité (QM) sanctionnée par des accréditations. Nous harmoniserons nos registres du cancer, faciliterons l';échange de données pour le contrôle de qualité, les analyses comparatives (« benchmarking ») et la recherche clinique et mettrons en place un système efficace de QM dans chacun de nos centres. D’autre part, nous développerons un logiciel de gestion globale des greffes de cellules souches hématopoïétiques facilitant la recherche et l’accréditation.
Action 3 – Accès aux traitements innovants (WP5). Nous visons à encourager l’innovation et à la transférer rapidement en clinique. Cette action développera des itinéraires de soins et guidelines et les partagera avec des institutions associées. De plus, afin de faciliter l';accès aux traitements innovants, nous développerons une plate-forme commune de recherche par essais cliniques (CT) incluant une stratégie concertée sur les cancers rares.
Principaux résultats attendus:
Chaque action décrit les résultats attendus (C.1.2) et délivrables des WP 3-4-5. Une attention particulière sera portée à la durabilité des outils (plateforme des CT et itinéraires de soins, outil de gestion des greffes, outil PROM, registre du cancer, QM) pour que les partenaires bénéficient de processus améliorés, développent la recherche et respectent les exigences réglementaires via des accréditations.
La durabilité et la valeur ajoutée de notre collaboration augmenteront par transfert de divers outils (réseau d’HAD, téléconsultation, registres du cancer, itinéraires de soins et plateforme d’essais cliniques) à des institutions associées ainsi que par les échanges avec les registres nationaux/régionaux (eg LKR in NRW), créant ainsi un réseau synergique transfrontalier de lutte contre le cancer.
La perception de la qualité des soins par les patients dépend d’une bonne communication avec les acteurs de la santé. La facilitation de prises de décision personnalisées, l’évaluation par les patients, l’accès à des thérapies innovantes et l’HAD seront perçus par les patients comme de grandes avancées. Dans le WP2 (Communication), nous réaliserons une communication transparente des résultats aux parties prenantes (chercheurs, associations de patients, médecins généralistes, professionnels de l';oncologie) et au grand public. Nous solliciterons aussi un feed-back des patients, des groupes de soutien, des chercheurs, des professionnels de la santé et des décideurs.

Krebserkrankungen sind eine zunehmende Herausforderung. Trotz entscheidender Fortschritte in der Prävention und Behandlung nehmen aufgrund des demographischen Wandels und der überproportionalen Zunahme im Alter die Krebsinzidenz und –prävalenz zu. Neben den klassischen Therapien haben sich in den letzten Jahren zahlreiche innovative Behandlungsoptionen wie die Immuntherapie und die molekular zielgerichtete Therapie (sogenannte „personalisierte Krebstherapie“) eröffnet. Diese Therapien führen zu einer Verbesserung des Überlebens bei gleichzeitig reduzierter Belastung durch die Therapie. Man spricht hier von einer Chronifizierung von Krebserkrankungen.
Vor diesem Hintergrund haben sich gemeinsam mit den Universitäten klinische Experten-Zentren, sog. „Comprehensive Cancer Centers (CCC)“ gebildet, die sich mit speziellen integrierten multidisziplinären Versorgungskonzepten annehmen. Kernelemente dieser Strukturen sind die Verbindung von qualitätsgesicherter, hochinnovativer Versorgung (inkl. klinischer und translationaler Forschung) und die Verfügbarmachung dieses medizinischen Fortschritts auch in weniger dicht besiedelte Bereiche durch die Vernetzung mit Kliniken und Praxen (sog. „Outreach“-Programme).
Unsere beteiligten Krankenhäuser stellen sich dem Auftrag einer optimalen Versorgung von Krebspatienten in der Euregio durch eine ständige Verbesserung des Angebotes. Der Austausch zwischen den in diesem Antrag grenzübergreifend zusammengeschlossenen akademischen Zentren sowie einem Privatunternehmen wird synergistische Lösungen schaffen, mit denen der Zugang von Krebspatienten in der Euregio zu innovativen Therapiekonzepten der personalisierten Krebsmedizin verbessert werden kann.
Ziele des Projektes und Aktivitäten:
OncoCare hat sich zum Ziel gesetzt, die Qualität der Versorgung von Krebspatienten in der Euregio zu verbessern. Dies erfolgt durch 3 Arbeitspakete (sog. Work Packages, WP 3-4-5), die jeweils gut vorbereitete und realistische Teilprojekte (TP, sog. „Actions“) enthalten, welche die Neuerungen in der Organisation der Versorgung und den Zugang zu innovativen Therapien ermöglichen.
Patienten-zentrierte Medizin (WP3): TP 3-1 ermöglicht durch Nutzung eines Teleconsulting-Systems den Austausch medizinischer Expertise in einem sich rasch entwickelnden Feld und somit die Patienten-bezogene Diagnostik und Beratung. TP 3-2 dient der Entwicklung und Implementierung einer Smartphone-tauglichen, vom Patienten autonom bedienbaren Lebensqualitätserfassung (Patienten-bezogene Endpunkte, PROM). TP 3-3 hat die onkologische Betreuung der Patienten in ihrem unmittelbaren häuslichen Umfeld („ home hospitalisation “) zum Ziel.
Qualität der Versorgung (WP4): TP 4-1 zielt zunächst auf eine Harmonisation der Krebsregister, den Datenaustausch für Qualitätskontrolle, Benchmarking und klinische Forschung ab. In TP 4-2 erfolgt die Entwicklung einer Software für das Management der hämatopoietischen Stammzelltransplantation (HSCT) entwickelt, die die Forschung und die Akkreditierung erleichtert.
Transfer von Innovationen in die Klinik (WP5): Ziel ist die Implementierung von personalisierter und innovativer Krebsmedizin in der Euregio. Dazu gehört die gemeinsame Etablierung evidenz-basierter Behandlungspfade unter Einbeziehung innovativer Diagnostik- und Therapieangebote und deren Verfügbarmachung an die vernetzten Partnerinstitutionen (TP5-1). Zusätzlich entsteht eine Forschungsplattform für klinische Studien (CT), um den Zugang zu innovativen Therapien zu erleichtern und den Austausch und Abgleich bestehender klinischer Studienaktivitäten unter Berücksichtigung seltener Tumorerkrankungen und Subtypen häufiger Tumorerkrankungen zu ermöglichen (TP 5-2).
Wichtigste Ergebnisse & Nachhaltigkeit:
Die Zielerreichung und Nachhaltigkeit der Projekte ergeben sich aus der Implementierung der Instrumente (Behandlungspfade, PROM, klinische Studien, Abwicklung der Blut-Stammzelltransplantation, Krebsregister, QM) in den Institutionen aller Projektpartner, einschließlich der angeschlossenen Kliniken, Forschungseinrichtungen und Praxen. Die grenzüberschreitende Zusammenarbeit der regionalen Partner und der Abgleich der angewendeten Methoden und Ergebnisse mit den regionalen Krebsregistern (z.B. dem Landeskrebsregister NRW in Deutschland) wird durch Synergieeffekte zu einer Verbesserung der Versorgungssituation von Tumorpatienten in unserer Region führen.
Die Wahrnehmung der Patienten hinsichtlich der Qualität der Gesundheitsversorgung hängt in hohem Maße von der Qualität der Kommunikation mit Gesundheitspersonal ab. Diese Qualitätseinschätzung wird verbessert durch den erleichterten Zugang zu evidenz-basierter personalisierter Medizin und innovativen Therapien, die direkte Einbeziehung der Patienten sowie die Möglichkeit der vereinfachten Versorgung im häuslichen Umfeld. Im Rahmen von WP2 (Kommunikation) wird das Gesamtprojekt interessierten Gruppen und Versorgern angeboten (Patientenselbsthilfegruppen, Hausärzten, Forschern, Onkologen…). Es werden Rückmeldungen zur Bewertung der Resultate der Teilprojekte und des Verbundes eingeholt.

1. Home hospitalization: Oncocare managed more than 7.300 treatment visits at home (chemotherapy, antibiotics, physical therapy…), improving patient comfort and freeing hospital beds.
2. International guidelines: Oncocare enabled the development of numerous standardized care itineraries and benchmarking, and facilitated the obtaining of international accreditations.
3. Hematopoietic stem cell transplantation (HSCT) management software: Oncocare developed an IT tool for managing all aspects of hematopoietic stem cell transplantation.

The aim of the project is to facilitate cross-border functioning in oncology for citizens and oncology centers in the Meuse-Rhine Euregio: Teleconsulting, Home Hospitalization, Patient-Reported Outcome Measure (PROM), Harmonized Cancer Registries Enabling Cross-Border Benchmarking, Joint Clinical Trial Management Platform.
The net result will be: (1) a reduction in patient travel despite improved and diversified care; (2) increased cross-border mobility of care staff.

Le projet a comme objectif de faciliter, pour les citoyens et les centres d’oncologie de l’Euregio Meuse-Rhin, le fonctionnement transfrontalier en oncologie : Téléconsulting, Hospitalisation à domicile, Patient-Reported Outcome Measure (PROM), Registres du cancer harmonisés permettant le Benchmarking transfrontalier, Plateforme commune de gestion des essais cliniques.
Le résultat net sera : (1) une diminution des déplacements de patients malgré une offre de soins améliorée et diversifiée ; (2) une augmentation de la mobilité transfrontalière du personnel de soins.

Das Projekt hat zum Ziel die onkologische Versorgung in der Grenzregion zu vereinfachen und zu verbessern und diese Effekte sollen für die Patienten und für die onkologischen Zentren der Euregio Maas-Rhein greifbar sein: Teleconsulting, Patienten-bezogene Endpunkte (PROM), Heimversorgung von Krebspatienten (HAD), grenzüberschreitende Angleichung der Krebsregister & Benchmarking, Software zur Organisation der Stammzelltransplantation, Angleichung von Behandlungspfaden, gemeinsame Plattform für Durchführung von klinischen Studien.
Ein Nettoergebnis des Projektes ist (1) die Verringerung von beschwerlichen Anreisen bei Verbesserung und Erweiterung der Versorgungsmöglichkeiten; (2) Eine Zunahme der grenzüberschreitenden Mobilität des Pflegepersonals.
