Description (FR): Connected Communities (CC) relève le défi social du vieillissement de la population (+ de 65 ans au RU, 60 en FR) face à des budgets d’action sociale incapables de s’aligner sur la croissance du nombre de seniors. Ceux-ci exigent une aide plus longtemps et souffrent de maladies plus longues. La solution innovante ASP+ proposée veut soutenir les personnes âgées isolées, ou risquant l’isolement, en améliorant leur accès aux équipements et services de santé et de bien-être. Les coûts annuels de l’isolement dans les communautés sont estimés à 32 milliards de £ au RU et 39,4 milliards d’€ en Fr en dépenses de santé, perte de productivité et augmentation de sollicitation des services publics et sociaux (CEBR 2017) (Service Public Français 2018). CC propose de lutter contre ce déficit de ressources en dirigeant les seniors vers les services des communautés de la zone FMA, améliorant ainsi la capacité des seniors à se prendre en charge, à être plus heureux, à réduire leur solitude et leur recours aux services sociaux.
Sur 10 Actions Haut Impact (HIT) évaluées, le “Royal College” des généralistes estime que le recours à l’ASP+ (pratique consistant à orienter les patients vers des soins non-médicaux) est une des solutions les plus efficaces et bénéfiques pour les patients et équipes de médecins. L’approche ASP « + que le médical » de CC, qui comprend traditionnellement un éventail d’activités (cours de danse, clubs de cuisine, etc.) ira au-delà des programmes traditionnels, offrant aux seniors un accès aux services locaux adéquats pour améliorer leur santé et leur bien-être. Une fois la demande identifiée, ces services sont créés s’ils manquent ou si le trajet est trop long. Ce programme est unique au regard de l’éventail des services. Ont fourni des attestations de soutien : services de santé, associations, gouvernement et police.
Agir en amont et construire une coopération entre professionnels compétents et bénévoles bien préparés est impératif. Une stratégie sera élaborée pour appuyer la formation des bénévoles et surtout en Fr, renforcer la collaboration avec les associations grâce au retour d’expérience du RU. Parallèlement à la livraison du modèle, une série d’outils sera élaborée grâce aux expériences répertoriées dans les deux pays, aidant ainsi d’autres zones à utiliser la même approche applicable aussi à d’autres groupes cibles. Seul un tiers des généralistes (31%) a déclaré savoir conseiller les patients (The Low Commission, 2015) qui évoquent des sujets non médicaux. Nesta a interviewé 2.000 particuliers. Plus de la moitié (55%) déclare souhaiter que leur médecin propose des ordonnances sociales et participatives.
CC permettra aux seniors de mieux gérer leur propre santé et besoins sociaux grâce au soutien adéquat pour les aider à rester indépendant plus longtemps. Amélioration du bien-être et indépendance évite ou retarde la nécessité d’une intervention plus lourde des services institutionnalisés de santé et soins.

The social prescribing plus innovative solution that it proposes will support older isolated people, or those at risk of being isolated, by improving their access to health and well-being facilities and services. It is estimated that community disconnectedness costs £32 billion in the UK and €
39.4 billion in France annually in healthcare costs, lost productivity, and increased demand on social and public services (CEBR 2017) (French Public Service 2018). Connected Communities proposes to combat this loss in resources by connecting older people and services within FCE communities, thereby boosting their ability to help themselves, increase happiness, decrease loneliness and reduce social services demand.
Of 10 High Impact Actions evaluated, the Royal College of GPs found that the recommendation to utilise social prescribing – the practice of referring patients to non-medical care - to be one of the most effective and beneficial for both GP teams and patients. Connected Communities’ "more than medicine" approach of social prescribing, which typically includes activities from dance classes to cookery clubs will go further than traditional schemes, travelling out to older people to link them to appropriate locally developed services to improve their health and wellbeing. Services will be commissioned where demand is identified and they do not exist or are too far to travel to. It is also unique in the range of services that will be brought together. Health organisations, voluntary groups, government and the police have all provided statements of support.
It is imperative to act early and build up co-operation between skilled professionals and well-prepared volunteers. A strategy will be developed to support volunteer’s training and strengthen collaboration with the voluntary sector, particularly in France, learning from the UK. As the model is delivered a set of tools will be developed based on experiences to date in both countries. It will help other areas follow the same approach, which could equally be taken with other target groups. When patients raised non-health issues, only one third (31 per cent) of GPs said they advise patients adequately themselves (The Low Commission, 2015). Nesta questioned 2,000 members of the public. More than half (55%) said they would like their GP to offer social prescriptions.
Connected Communities will enable older people to better manage their own health and social care with the right support when needed to help them be more independent for longer. Improved wellbeing and independence will prevent or delay the need for some formal health and wider care services.
