Description (NL): Een aanzienlijk deel van de oudere bevolking in landelijke gebieden van de grenszone lijdt aan chronische ziekten, die tot een functionele achteruitgang en een behoefte aan gecoördineerde zorg kunnen leiden. Het project HIS2R, gericht op de Zoast des Ardennes, heeft tot doel de gezondheid van chronische patiënten, het comfort en de samenwerking van zorgverleners te verbeteren en de kosten voor de patiënt en de sociale zekerheid te verlagen. Het heeft bovendien tot doel de coördinatie van de grensoverschrijdende zorgverlening in landelijke gebieden te optimaliseren, met de ondersteuning van nieuwe technologieën (verbonden voorwerpen en efficiënte en veilige informatieoverdracht). Patiënten met chronisch hartfalen of chronische obstructieve longziekte worden uitgerust met een apparaat voor het veilig verzamelen en verzenden van gezondheidsinformatie en waarschuwingen aan zorgverleners. Dankzij een systeem van telebewaking en casemanagers kan in realtime op de waarschuwingen worden gereageerd. De gegevens van de patiënt kunnen aan beide zijden van de grens worden geraadpleegd.
Read more Description (EN): A significant proportion of the elderly population in cross-border rural areas suffer from chronic diseases that can lead to functional decline and a need for coordinated care. The HIS2R project, focused on the Ardennes region, aims to improve the health of chronic patients, the comfort and collaboration of providers, and reduce costs for patients and social security. It also aims to optimise cross-border coordination of care in rural areas, with the support of new technologies (connected objects and efficient and secure transmission of information). Patients with chronic heart failure or chronic obstructive bronchopathy are equipped with a device to collect and transmit health information and alerts in a secure manner to caregivers. A system of televigilance and case managers makes it possible to react to alerts in real time. Patient data can be viewed on both sides of the border.

Le projet HIS2R, centré sur la Zone des Ardennes, vise à améliorer la santé des patients chroniques, le confort et la collaboration des prestataires, à réduire les coûts pour le patient et pour la sécurité sociale. Il vise en outre à optimiser la coordination des soins à l'échelle transfrontalière en zone rurale, avec l'appui de nouvelles technologies (objets connectés et transmission efficace et sécurisée de l'information). Les patients souffrant d'insuffisance cardiaque chronique ou de bronchopathie chronique obstructive sont équipés d'un dispositif permettant de recueillir et transmettre des informations de santé et alertes, de manière sécurisée aux soignants. Un système de télé-vigilance et case managers permet de réagir en temps réel aux alertes. Les données du patient sont consultables de part et d'autre de la frontière.
Read more Achievements (EN): During the first 2 years of the HIS2R project, all the operators established a cross-border working process and a strong Franco-Belgian cooperation was established despite the exit and entry of several partners during the project and the Covid-19 pandemic, which mobilised enormously the operators involved in the management of the health crisis. In terms of communication, a visual identity has been developed and disseminated through communication tools aimed at both the general public and specialists. Communications were made at various events on both sides of the border, for example at the Congress of Gerontology and Geriatrics in Liège, at the Jetsan Days in Troyes, during a videoconference with the INTERREG V-A Northern Ireland-Ireland-Scotland project. A website (https://www.his2r-interreg.eu/), a Facebook page and a LinkedIn group have been put online and are regularly fed by the project’s stakeholders. From the point of view of the framework, two research protocols have been validated by French and Belgian ethics committees. A patient information and consent note was also developed. On the GDPR side, an impact assessment and a CNIL file have been validated. Various collections of literature related to the project and an article on the questioning of remote monitoring practices of chronic diseases in rural areas were produced in order to contextualise our study. Several cross-border focus groups with representatives of caregivers and caregivers, as well as an online survey, and interviews with patients were conducted to define the framework of the project and to align it as closely as possible with the needs of the field. As a result of the delay on the GDPR file and the significant impact of the Covid-19 outbreak, the patient’s medical follow-up period has been reduced from 12 to 6 months. At the operational level, the substantial work of preparing the device concerned the development of the course of the study and the decision-making algorithm, the search for a case manager and its training, the training of televigilance personnel, the awareness of general practitioners and specialists, the search for patients corresponding to the criteria for inclusion at the level of the partner hospitals. The long and difficult recruitment of patients made it possible to equip 21 patients ((including 14 Belgians, 5 French patients treated in Belgian hospital and 2 French) of a sized tablet and televigilance provided by Telepronam (follow-up H24). The follow-up of patients engaged in daily use of health parameters and who benefited from 5 therapeutic education sessions with the nurse case manager took place from July 1 to December 31, 2021. A video interview of a French patient and the nurse case-manager was posted on social media and the project’s website. The quantitative and qualitative results included in a case report form discussed in the team gave rise to an evening discussion with specialists and general practitioners with a patient in the study as well as an article at the European Geriatric Medicine Society EUGMS. A brochure presenting the results was given to patients, their entourage and published on the project website. Home monitoring allows the identification of patients in need of therapeutic adjustments (12 patients participating in the project have benefited). Patients find that their general practitioner made little use of the possibilities offered by the device, such as summary diagrams or the digital notebook. In summary, remote monitoring appears to be a promising solution for the follow-up of patients whose health status requires therapeutic adjustments. An increase in the motivation to take care of themselves has been observed in patients. Technology must be improved and put at the service of the patient, not the other way around. The system must move towards more mobility, connectivity and personalisation: parameters, alerts, and therapeutic education in various areas such as vaccination, physical activity, depression, management of other problems and pathologies (ability to walk, chew, see and hear, diabetes, kidney pathologies, etc...). The case manager function is the key to managing alerts, therapeutic education and patient coaching. The case manager was seen as a confidant who helps overcome isolation. Many patients were in psycho-social difficulties and their mental health was affected. We have also been confronted with the refusal of the most complex patients (too fragile, unconnected, etc.) and this raises the question of deepening social inequalities. As far as ethical, ethical and regulatory issues are concerned, exchanges have been organised between leading telemedicine players such as Pulsy, FRATEM/RSW and a French specialist in health and e-health management. Many questions remain outstanding such as how we can identify a French patient in Belgium and vice versa or the way in which a French professional can be identified in Belgium and vice versa. In order for data sharing to take place, a patient and professional identification system must be recognised and ideally “certified”. The key function of multifaceted casemanager on both sides of the border led us to organise a seminar at the CHU in Belair. It addressed existing arrangements and training needs. In conclusion, the closing event of 28 June in Viroinval allowed to present the results of the project and to discuss certain topics related to HIS2R, such as the ethical issues of digital health and its evolution at cross-border and European level.
Read more Achievements (FR): Durant les 2 premières années du projet HIS2R, l’ensemble des opérateurs a établi un processus de travail transfrontalier et une solide coopération franco-belge s’est installée malgré la sortie et l’entrée de plusieurs partenaires en cours de projet et la pandémie Covid-19 qui a mobilisé énormément les opérateurs engagés en première ligne dans la gestion de la crise sanitaire. En matière de communication, une identité visuelle a été élaborée et diffusée à travers des outils de communication destinés tant au grand public qu’aux spécialistes. Des communications ont été réalisées lors de différents événements de part et d’autre de la frontière, par exemple au Congrès de Gérontologie et Gériatrie à Liège, aux journées Jetsan de Troyes, lors d’une visioconférence avec le projet INTERREG V-A Northern Ireland-Ireland-Scotland. Un site internet (https://www.his2r-interreg.eu/ ), une page Facebook ainsi qu’un groupe LinkedIn ont été mis en ligne et sont régulièrement alimentés par les acteurs du projet. Du point de vue du cadre, deux protocoles de recherche ont été validés par des comités d’éthique français et belge. Une note d’information et de consentement du patient a également été élaborée. Côté RGPD, une analyse d’impact et un dossier CNIL ont été validés. Différents recueils de littérature en lien avec le projet et un article sur le questionnement relatif aux pratiques de télésurveillance des maladies chroniques en milieu rural ont été produits afin de contextualiser notre étude. Plusieurs focus groups transfrontaliers regroupant des représentants d’aidants proches et prestataires de soins ainsi qu’un sondage en ligne, et des entretiens avec des patients ont été réalisés afin de définir le cadre du projet et de le faire correspondre le plus possible aux besoins du terrain. Suite au retard accumulé sur le dossier RGPD ainsi qu’à l’impact important de l’épidémie Covid-19, la période de suivi médical des patients a été réduite de 12 à 6 mois. Au niveau opérationnel, le travail conséquent de préparation du dispositif a concerné l’élaboration du déroulement de l’étude et de l’algorithme décisionnel, la recherche d’une casemanager et sa formation, la formation du personnel de télévigilance, la sensibilisation des médecins généralistes et spécialistes, la recherche de patients correspondant aux critères d’inclusion au niveau des hôpitaux partenaires. Le long et difficile recrutement des patients a permis d’équiper 21 patients (dont 14 belges, 5 français soignés en hôpital belge et 2 français) d’une tablette dimensionnée et de la télévigilance assurée par Telepronam (suivi H24). Le suivi des patients engagés dans une prise quotidienne des paramètres de santé et qui ont bénéficié de 5 séances d’éducation thérapeutique avec l’infirmière case-manager s’est déroulé du 1er juillet au 31 décembre 2021. Une vidéo interview d’un patient français et de l’infirmière case-manager’ a été diffusée sur les réseaux sociaux et le site web du projet. Les résultats quantitatifs et qualitatifs repris dans un case report form débattus en équipe ont donné lieu à une soirée débat avec les médecins spécialistes et généralistes ayant un patient dans l’étude ainsi qu’un article à l’European Geriatric Medicine Society EUGMS. Une brochure de présentation des résultats a été remise aux patients, à leur entourage et publiée sur le site internet du projet. Le monitoring à domicile permet l'identification de patients à besoin d'ajustements thérapeutiques (12 patients participant au projet en ont bénéficié). Les patients constatent que leur médecin généraliste utilisait peu les possibilités offertes par le dispositif comme par exemple les diagrammes récapitulatifs ou le cahier digital. En résumé, la télésurveillance apparaît comme une solution prometteuse pour le suivi des patients dont l’état de santé nécessite des ajustements thérapeutiques. Une augmentation de la motivation à bien se prendre en charge a été constatée chez les patients. La technologie doit être améliorée et mise au service du patient et non l’inverse. Le système doit évoluer vers plus de mobilité, de connectivité et de personnalisation: paramètres, alertes, et l’éducation thérapeutique dans différents domaines tels la vaccination, l’activité physique, la dépression, la prise en charge des autres problèmes et pathologies (capacité de marcher, de mastiquer, de voir et entendre, diabète, pathologies rénales, etc…). La fonction de case-manager est la clef de la gestion des alertes, de l'éducation thérapeutique et du coaching du patient. La gestionnaire de cas a été perçue comme une confidente qui aide à vaincre l’isolement. De nombreux patients se trouvaient dans des difficultés psycho-sociales et leur santé mentale en était affectée. Nous avons aussi été confrontés au refus des patients les plus complexes (trop fragiles, non connectés, etc.) et ceci pose la question du creusement des inégalités sociales. En ce qui concerne les questions éthiques, déontologiques et règlementaires a organisé des échanges entre des acteurs de premier plan de la télémédecine comme Pulsy, la FRATEM/ RSW et un spécialiste français du management de santé et e-santé. Beaucoup de questions restent en suspens telles que la manière dont nous pouvons identifier un patient français en Belgique et vice-versa ou encore celle par laquelle un professionnel Français peut être identifié en Belgique et vice-versa. Pour que le partage de données puisse se faire, un système d’identification des patients et des professionnels doit être reconnu et idéalement « certifié ». La fonction clé de casemanager multiforme de part et d’autre de la frontière nous a mené à organiser un séminaire au CHU de Belair . Il a traité des dispositifs existants et des besoins en formation . En conclusion, l’événement de clôture du 28 juin à Viroinval a permis de présenter les résultats du projet et d’échanger autour de certaines thématiques en lien avec HIS2R, comme les enjeux éthiques de la santé numérique et l’évolution de celle-ci au niveau transfrontalier et européen.

Expected Results (NL): De verwachte voordelen voldoen aan 5 volksgezondheidsdoelstellingen: impact op de gezondheid, autonomie, ontwikkeling van zorgtrajecten, bedrijfsmodel en grensoverschrijdende uitwisseling van gezondheidsgegevens:
• Vermindering van onnodige ziekenhuisopname, verplaatsingen, reactietijd in geval van problemen, ondersteuning en empowerment (patiënt, familieleden en zorgverleners rondom de patiënt), verbeterde therapietrouw (geïntegreerde zorgtrajecten), preventie van complicaties (frequentie, duur en ernst);
• Verkenning van de economische levensvatbaarheid van het systeem (vergoeding en vergelding), de aanvaardbaarheid (deontologie, ethiek), technische en juridische haalbaarheid, mogelijkheid van generalisatie).
• Ontwikkeling van een TF-gezondheidsgebied en een TF-informatieoverdrachtsysteem; gezamenlijke antwoorden op gemeenschappelijke vragen (ruraliteit, tekort aan dienstverleners, ...), gedeelde TF-voorzieningen (24/24 bijstand, case managers enz.)
In een eerste fase zullen de diensten, verstrekt door de partners, een steekproef van 800 personen ten goede komen. Ontwikkeling van onder meer een telebewakingsdienst, apps voor smartphone/tablet en een grensoverschrijdend platform voor uitwisseling van medische informatie. Minstens 50 patiënten zullen worden uitgerust met tablets en zullen door een case manager worden begeleid. 200 van hen zullen een gepersonaliseerde coaching krijgen.
