Chaque année, 260 enfants ou bébés atteints d’une malformation cardiaque auraient besoin de bénéficier d’une intervention chirurgicale hyperspécialisée. Or, il n’existe à l’heure actuelle pas de prise en charge de chirurgie cardiaque infantile sur la zone transfrontalière France-Wallonie-Vlaanderen. Le CHU de Lille souhaite relever ce défi de santé publique en proposant cette prise en charge à l’horizon 2020. Le développement cette activité de chirurgie cardiaque infantile vise à améliorer la qualité de vie de la population transfrontalière, notamment en réduisant les risques liés aux transferts actuels de ces nouveau-nés vers Paris et Bruxelles, pour des durées indéterminées et parfois longues pour les familles loin de leur lieu de vie. Le CHU de Lille s’associe à l’un des 3 centres belges en cardiologie pédiatrique pour initier l’apprentissage de ses professionnels afin d’atteindre le niveau d’expertise requis. Le cycle de formation concerne 28 médecins et 48 paramédicaux français qui interviennent aux côtés de leurs collègues belges pour la prise en charge de patients, y compris français. Les deux établissements visent également à créer un pôle européen d’excellence et de recherche conjoint sur les cardiopathies congénitales. Le réseau de compétence ainsi formé inclue dans son maillage les établissements de santé du versant belge, qui bénéficient de la dynamique de diffusion des connaissances. Celui-ci bénéficie également de la dynamique d’emploi suscitée par la création de cette activité nouvelle au CHU de Lille.

Every year, 260 children or babies with heart defects are in need of hyperspecialised surgery. However, there is currently no infrastructure for infant cardiac surgery in the France-Wallonia-Flanders cross-border area. Lille University Hospital wishes to meet this public health challenge by offering this care by 2020. The development of this infant cardiac surgery activity aims to improve the quality of life of the cross-border population, in particular by reducing the risks associated with the current transfers of these newborns to Paris and Brussels, for indefinite and sometimes long periods for families far from their place of residence. The University Hospital of Lille is partnering with one of the 3 Belgian centres in paediatric cardiology to launch the training of its professionals in order to reach the required level of expertise. The training course involves 28 French doctors and 48 paramedics who work alongside their Belgian colleagues to provide care for patients, including French patients. The two institutions also aim to create a European centre of excellence and joint research on congenital heart disease. The network of competence thus formed includes the health establishments on the Belgian side in its network, which benefit from the dynamics of knowledge dissemination. It also benefits from the employment dynamics generated by the creation of this new activity at Lille University Hospital.

Jaarlijks zouden 260 kinderen of baby’s met een hartafwijking een hypergespecialiseerde chirurgische ingreep nodig hebben. Vandaag bestaat er geen pediatrische hartchirurgiezorg in de grenszone Frankrijk-Wallonië-Vlaanderen. Het CHU van Rijsel wil deze uitdaging voor de volksgezondheid aanpakken en deze zorgverlening tegen 2020 aanbieden. De ontwikkeling van de pediatrische hartchirurgie is bedoeld om de levenskwaliteit van de grensbevolking te verbeteren, met name door de risico’s van de huidige overbrenging van pasgeboren kinderen naar Parijs en Brussel, te verminderen. Deze overbrenging is voor onbepaalde duur en houdt voor de familie vaak verre reizen in. Het CHU van Rijsel werkt samen met een van de drie Belgische centra voor pediatrische cardiologie om de opleiding van zijn zorgverleners op te starten teneinde het vereiste niveau van deskundigheid te bereiken. De opleidingscyclus betreft 28 artsen en 48 paramedici uit Frankrijk, die samen met hun Belgische collega’s zullen instaan voor de zorg van patiënten, onder wie ook Franse patiënten. Beide instellingen willen ook een gemeenschappelijk Europees expertise- en onderzoekscentrum voor aangeboren hartafwijkingen in het leven roepen. De gezondheidsinstellingen aan de Belgische maken deel uit van dit competentienetwerk en genieten zodoende mee van de dynamiek van kennisverspreiding. Dit bevordert ook de tewerkstellingsdynamiek die de creatie van deze nieuwe activiteit in het CHU van Rijsel veroorzaakt.

Ce projet visait l'implantation d'une activité de chirurgie cardiaque néonatale et infantile au CHU de Lille à l'automne 2020 en s'appuyant sur les compétences reconnues des équipes des Cliniques Universitaires Saint- Luc (CUSL) de Bruxelles. Chaque année, 250 enfants ou nouveau-nés des Hauts-de-France atteints d'une malformation cardiaque ont besoin d'une intervention chirurgicale hyperspécialisée. Alors qu'il n'existait jusqu'alors aucune offre de soins en chirurgie cardiaque infantile sur la zone transfrontalière France-Wallonie-Vlaanderen, le CHU de Lille s’est donné comme objectif de pouvoir prendre en charge ces patients. Le CHU de Lille, en s'associant aux CUSL de Bruxelles, l'un des trois centres de cardiologie pédiatrique belges, a permis de former jusqu'à présent 15 médecins et 43 paramédicaux français afin d'atteindre le niveau de qualification requis. Les professionnels lillois en formation à Bruxelles participent à la prise en charge de patients français et belges en s'appuyant sur l'expertise reconnue des équipes médicales et soignantes des CUSL. A ce jour, 154 patients français ont été pris en charge aux CUSL dans le cadre de ce programme. Grâce au compagnonnage de Saint-Luc, le CHU de Lille propose désormais une activité de chirurgie cardiaque infantile qui contribue à améliorer la qualité de vie de la population transfrontalière, notamment en réduisant les risques liés aux transferts actuels de ces nouveau-nés vers Paris et en favorisant un accès à une prise en charge de proximité pour les familles. La première intervention a été réalisée avec succès le 15 octobre 2020. Le Directeur Général de l'ARS Hauts-de-France a tenu à saluer ce projet d’envergure dans le cadre d’une visite au CHU de Lille le 2 décembre 2020 afin d’échanger avec les équipes et de visiter les installations. Malgré la crise sanitaire liée au Covid-19, la prise en charge des enfants des Hauts-de-France par les CUSL de Bruxelles a été maintenue a minima et l'ensemble des travaux a pu être achevé avec un retard de trois semaines seulement. Du 15 octobre 2020 au 31 mars 2022, 83 patients ont été pris en charge à Lille sur le plateau technique de Jeanne de Flandre. Depuis le 1er avril 2018, 243 patients ont bénéficié d’une ou de plusieurs interventions chirurgicales, soit sur le plateau technique des CUSL, soit sur celui du CHU de Lille. Début avril 2022 a été célébré le premier anniversaire de l’implantation de la chirurgie cardiaque pédiatrique au CHU de Lille. Les équipes des deux établissements collaborent pour mettre à disposition de nombreux outils de communication en français et en néerlandais à destination des enfants et des familles, des professionnels et du public (site internet dédié au projet, plaquettes d'information, outils ludiques et pédagogiques). Les deux établissements, qui ont pour objectif la création d’un pôle européen d'excellence et de recherche conjoint sur les cardiopathies congénitales, ont déjà posé les bases de cette future collaboration.

This project aimed at the implementation of a neonatal and infant cardiac surgery activity at the University Hospital of Lille in the autumn of 2020 based on the recognised skills of the teams of the Cliniques Universitaires Saint-Luc (CUSL) in Brussels. Every year, 250 children or newborns in Hauts-de-France with heart defects require hyperspecialised surgery. While until then there was no offer of care in childhood cardiac surgery in the cross-border zone France-Wallonie-Vlaanderen, the Lille University Hospital has set itself the objective of being able to take care of these patients. The University Hospital of Lille, by partnering with the Brussels CUSL, one of the three Belgian pediatric cardiology centres, has so far trained 15 French doctors and 43 paramedics in order to reach the required level of qualification. Lille professionals undergoing training in Brussels participate in the care of French and Belgian patients by relying on the recognised expertise of the medical and healthcare teams of the CUSL. To date, 154 French patients have been treated at the CUSL as part of this program. Thanks to the companionship of Saint-Luc, the CHU de Lille now offers an activity of infant cardiac surgery that helps to improve the quality of life of the cross-border population, in particular by reducing the risks associated with the current transfers of these newborns to Paris and by promoting access to local care for families. The first intervention was successfully carried out on 15 October 2020. The Director General of the ARS Hauts-de-France welcomed this major project as part of a visit to the University Hospital of Lille on 2 December 2020 in order to exchange with the teams and visit the facilities. Despite the COVID-19 health crisis, the care of the children of Hauts-de-France by the Brussels CUSL was kept to a minimum and all the work could be completed with a delay of only three weeks. From 15 October 2020 to 31 March 2022, 83 patients were treated in Lille on the technical plateau of Jeanne de Flandre. Since 1 April 2018, 243 patients have benefited from one or more surgical procedures, either on the technical plateau of the CUSL or on that of the Lille University Hospital. At the beginning of April 2022, the first anniversary of the implantation of pediatric cardiac surgery was celebrated at the CHU in Lille. The teams of the two institutions collaborate to make available many communication tools in French and Dutch for children and families, professionals and the public (website dedicated to the project, information leaflets, playful and educational tools). The two institutions, which aim to create a European pole of excellence and joint research on congenital heart disease, have already laid the foundations for this future collaboration.

Création d'une offre de service de prise en charge chirurgicale d'hyper-recours extrêmement spécialisée au CHRU de Lille. Formation de 28 médecins et 48 paramédicaux français au CUSL et supervision des premières interventions par deux médecins belges au CHRU de Lille en 2020. 408 personnes sont donc concernées par ce dispositif, dont 330 patients. Mise en commun des bases de données du CUSL et du CHRU de Lille ; publications de recherche centré sur l'étude des corrélations entre les anomalies de flux et différents acteurs de l'hémostase.

Creation of a highly specialised hyper-access surgical management service offer at Lille University Hospital. Training of 28 French doctors and 48 French paramedics at the CUSL and supervision of the first operations by two Belgian doctors at Lille University Hospital in 2020. Four hundred and eight people are therefore affected by this scheme, including 330 patients. Pooling of the databases of the CUSL and Lille University Hospital; research publications focused on the study of correlations between flow anomalies and different haemostasis stakeholders.

Creatie van een dienst voor zeer gespecialiseerde chirurgische zorgverlening in het CHRU van Rijsel. Opleiding van 28 artsen en 48 paramedici uit Frankrijk in de CUSL en toezicht door twee Belgische artsen tijdens de eerste operaties in het CHRU van Rijsel in 2020. Bij dit project zijn 408 personen betrokken, onder wie 330 patiënten. Gemeenschappelijk gebruik van de databases van de CUSL en het CHRU van Rijsel; onderzoekspublicaties toegespitst op de studie van de correlaties tussen afwijkingen van de bloedsomloop en verschillende actoren van de hemostase.

Eventually, a children's cardiac surgery service will be set up at Lille University Hospital. The two institutions also aim to create a European centre of excellence and joint research on congenital heart disease. The network of competence thus formed will include in its network the health establishments on the Belgian side of the ZOAST (Zone organised for access to cross-border care), which will benefit from the dynamics of knowledge dissemination. This will trigger a leverage effect in terms of innovation and research within the cross-border territory, as well as on the employment dynamics generated by the creation of this new activity at Lille University Hospital.

Implementatie, op termijn, van een dienst pediatrische hartchirurgie in het CHRU van Rijsel. Beide instellingen willen ook een gemeenschappelijk Europees expertise- en onderzoekscentrum voor aangeboren hartafwijkingen in het leven roepen. De gezondheidsinstellingen aan de Belgische zijde van de ZOAST zullen in het op deze wijze gevormde netwerk worden opgenomen en zodoende mee genieten van de dynamiek van kennisverspreiding. Het project zal een hefboomeffect teweegbrengen in termen van innovatie en onderzoek in het grensoverschrijdende gebied, evenals een door de creatie van deze nieuwe activiteit in het CHRU van Rijsel veroorzaakte tewerkstellingsdynamiek.
