Description (FR): A l’heure actuelle, les logiciels de reconnaissance d'images sont utilisés quotidiennement. Par exemple, les aéroports recourent à ces logiciels afin d'identifier un individu à partir des caractéristiques de son visage. Pourtant, chaque jour, des millions d’images médicales sont produites et analysées de manière manuelle et semi-quantitative. Toutefois, l’œil humain n’étant pas infaillible, une série d’informations lui échappent. L’imagerie médicale est devenue la clé de voûte de la médecine moderne puisqu’elle permet de repérer toutes sortes de maladies que l’œil humain ne peut détecter. Cependant, au vu des faiblesses de son analyse manuelle et semi quantitative, la création et le développement de l’approche “Radiomique » s'est avéré nécessaire. L’approche « Radiomique» est une technologie de rupture basée sur un logiciel original permettant l’extraction de plusieurs milliers de « signatures Radiomique » à partir d’une image médicale traditionnelle. En outre, une méthode se fondant sur le principe de l’ “apprentissage distribué”, conforme à la législation sur le respect de la vie privée des patients, et permettant une approche dite «Big data», a également été développée. Grâce à cette technologie, nous pouvons désormais partager des données médicales anonymes à travers le monde sans que l’on se trouve limité par les législations relatives à la protection de la vie privée. Cette technologie est capable d’analyser des milliers d’images médicales sans que celles-ci ne quittent l’hôpital d’origine en résolvant le problème relatif à la protection de la vie privée. Cette approche originale nous permettra de systématiquement améliorer et mettre à jour les signatures Radiomics développées et, ainsi, de consolider la position stratégique des PME eurorégionales impliquées. L'objectif de ce projet est d’assurer une commercialisation durable ainsi que la production d’outils radiomiques de manière continue - un concept né dans l’eurorégion, mais ayant désormais un impact dans le monde entier - ce qui permettra l’analyse d’images médicales standards pour le diagnostic. De plus, les signatures radiomiques seront intégrées dans des supports décisionnels multifactoriels pour les médecins ou pour les patients, facilitant de la sorte la médecine de précision, à la fois personnalisée et participative, et permettant de meilleurs soins de santé à un moindre coût. Les objectifs principaux sont triples 1. Mettre à jour et/ou installer la solution d'analyse d'images distribuée dans tous les hôpitaux participant au projet (WP1); 2. Développer et valider un flux de travail ("workflow") entre les participants, par ordre chronologique : analyses d'image médicales de manière distribuée sans que les images ne quittent l'hôpital d'origine, création de signatures radiomiques utiles en clinique, validation eurorégionale des signatures, intégration des signatures dans des produits potentiellement commercialisables (voir le point 3) (WP2). 3. Tester cinq types de produits potentiellement commercialisables parmi le réseau EURADIOMICS, à savoir : 3a. des signatures d'imagerie radiomiques potentiellement brevetables (WP3) 3b. des logiciels d'analyse d'images (WP2) 3c. des supports décisionnels pour médecins (WP4) 3d. des supports décisionnels pour patients ("Patient Decision Aid") (WP4) 3e. des modules d’e-apprentissage (WP5) Ce projet a également comme objectif plus spécifique de développer et de commercialiser la technologie des mégadonnées (dite “Big data" en anglais) portant sur le cancer de la tête et du cou, de l’œsophage, du poumon et du foie. Celle-ci se base sur des pétaoctets de données d’imagerie médicale stockées dans des hôpitaux du monde entier et les utilise afin de valider les systèmes d’aide à la prise de décisions et, sur base de ces derniers, d'améliorer le diagnostic et les traitements semi-automatisés du cancer. Le consortium est bien équilibré avec 4 PME (l’une étant le chef de file pour assurer la gestion de la commercialisation) et 4 hôpitaux (dont 3 universitaires). À la fin de ce projet, nous devrions avoir : 1. Des PME renforcées au sein d’ Eurorégion avec de nouveaux produits commerciaux et davantage de possibilités d’emploi 2. Des centres universitaires avec un rendement académique plus élevé, menant à de nouvelles subventions, y compris des bourses 3. De meilleurs soins procurés aux patients atteints d’un cancer de la tête et du cou, de l’œsophage, du poumon et du foie, basés sur une technologie rentable 4. Un réseau « gagnant-gagnant-gagnant » (faisant référence aux patients, aux PME et aux centres académiques), qui permettra un apprentissage distribué de l’imagerie médicale, et assurera la viabilité du projet.

Medical imaging has become the cornerstone of modern medicine, however many diseases cannot be detected by the human eye or properly assessed. Given the shortcomings in manual and semi-quantitative methods, it was necessary to develop an approach known as "Radiomics". Radiomics is a breakthrough technology based on a unique image software to extract and obtain very large amounts of quantitative image features from standard medical images and translate them into "Radiomics signatures". Moreover, it is a method that is based on the principle of "distributed learning", which respects the privacy laws concerning patient information, thereby enabling an approach called "Big Data". (Look at our animation). With this technology we can now share Radiomics knowledge with the world whilst being in full compliance with legislation on the protection of privacy. This technology enables the analysis of millions of medical images without the data physically leaving the hospital/clinic/institute, thereby resolving possible privacy protection issues. This innovative approach will allow us to systematically improve, update and further develop Radiomics signatures. This will strengthen the strategic position of the European regional partners concerned. The goal of this project is to continually provide sustainable marketing and product development for radiomics - a concept born in the Euroregion, but now with a global impact - and that the analysis of medical image will be applied in the clinic as a standard of care. Additionally, radiomics signatures will be integrated into multifactorial decision support for both physicians and patients for "precision medicine", both personal and participatory, resulting in better care at lower costs. The main goals are threefold: 1. Maintain and / or install the image analysis solution in all hospitals participating in the project (WP1); 2. To develop and validate a workflow between the participants, in chronological order: medical image analysis in a distributed manner without the images leaving the original hospital, development of clinically useful radiomics signatures, euregional validation of the signatures, integration of signatures in potentially marketable products (see point 3) (WP2). 3 test five types (a-e) of potential marketable products in the EURADIOMICS network, namely: a). potentially patentable radiomics signatures (WP3). b) (WP2) image analysis software; c). decision support systems for doctors (WP4); d). decision support systems for patients ( "decision tool") (WP4); e). e-learning modules (WP5). The use of large volumes of medical images will enable better treatments for cancer patients, primarily within the Euregion, and then worldwide. This project also has a more specific objective: the development and marketing of "big data" technology within the fields of head and neck cancer and cancer of the esophagus, lungs and liver. Harnessing the petabytes of medical image data stored in hospitals all over the world, decision support systems can be validated to improve the diagnosis and semi-automatic treatment of cancer. The consortium is well balanced with four SMEs (one leader for managing marketing) and four hospitals (including three universities). Objectives of this project: 1 SMEs in the Euregion strengthened with new commercial products and more jobs; 2 academic centers with higher academic performance, this leads to new grants. 3. better care for patients with head and neck, esophagus, lung and liver cancer, based on a cost-effective technology; 4. a 'win-win-win' Network (referring to patients, SMEs and academic centers), which enables medical imaging-analysis for distributed learning. Dennoch werden Millionen von medizinischen Bildern, die täglich erstellt werden, nur manuell und semi-quantitativ analysiert. Allerdings ist das menschliche Auge nicht unfehlbar und zahlreiche Informationen können der Aufmerksamkeit entgehen. Medizinische Bildgebung ist zu einem der Eckpfeiler der modernen Medizin geworden, weil viele Krankheiten, nicht vom menschliche Auge erkannt werden können. Aufgrund der Mängel der manuellen und semi-quantitativen Analyse, ist die Entwicklung von Radionomics notwendig. “Radiomics” ist eine revolutionäre Technologie, die auf einer Software zum Extrahieren von mehreren Tausend sogenannten "radionomischen Signaturen” innerhalb medizinischer Bildern basiert. (eine Erläuterung der radionomischen Signaturen ist auf der Website www.radiomics.org finden). Darüber hinaus ist Radiomics ein Verfahren, das auf dem "Distributed Learning"-Prinzip basiert - einem "Big Data" Ansatz, der zugleich die Privatsphäre der Patienten respektiert. Mit dieser Technologie können anonyme medizinische Daten, die weltweit veteilt sind, unter Beachtung der Rechtsvorschriften über den Schutz der Privatsphäre verwendet werden. Distributed Learning ist für die Analyse von Tausenden von medizinischen Bildern geeignet, ohne dass diese das Krankenhaus verlassen müssen. Dieser innovative Ansatz ermöglicht es, Radiomics Signaturen - basierend auf Tausenden von weltweit verteilten Datensätzen - zu entwickeln und systematisch zu verbessern, und damit die die strategische Position der beteiligten KMUs im europäischen Raum zu verbessern. Das Ziel dieses Projektes ist es, eine nachhaltige Kommerzialisierung von Radiomics-Software und Dienstleistungen zu erreichen und somit einen neuen Standard in der Medizin bei der Analyse von medizinischen Bildern zu etablieren – eine europäische Idee mit globlem Einfluss und Wirkung. Die Radiomics Signaturen können in den medizinischen Entscheidungsprozess von Ärzten und Patienten integriert werden. Dies wird sowohl eine personalisierte als auch eine partizipative Precision Medicine ermöglichen und verbessern. Gleichtzeitig verfügt Radiomics über ein enmormes Potential die Kosten des Gesundheitswesens zu reduzieren. Hauptziele des Projekts 1. Verbesserung und/oder Etablierung der Radiomics-Bildanalyse in allen im Projekt teilnehmenden Kliniken (WP1); 2. Entwicklung und Verifizierung eines Workflows zwischen den Teilnehmern, in chronologischer Reihenfolge: Analyse von medizinischen Bildern ohne dass diese Bilder die ursprüngliche Klinik verlassen müssen. Erarbeitung von klinisch nützlichen Radiomics Signaturen. Validierung der entwickelten Signaturen in Europa. Integration dieser Signaturen in potentiell marktfähige Produkte (siehe Punkt 3) (WP2). 3. Testen von fünf poteniell vermarktbaren Produkten innerhalb des EURADIOMICS Netzwerks: 3a. Patentierbare Radiomics Signaturen (WP3). 3b Bildanalysesoftware (WP2); 3c. Decision Support Software für Ärzte (WP4); 3d. Decision Support Software für Patienten (WP4); 3e. E-Learning-Module (WP5). Innerhalb dieses Projektes sollen große Mengen von medizinischen Bilddaten dazu genutzt werden, um Patienten mit Tumoren im Kopf-Hals-Bereich, der Speiseröhre, der Lunge und der Leber besser zu behandeln. Das Prinzip beruht darauf, Peta-Bytes medizinischer Daten, die derzeit weltweit in Krankenhäusern vorhanden sind und riesige Mengen bisher nicht genutzter Informationen enthalten, zur Entscheidungsfindung heranzuziehen, um in einer halbautomatisierten Form die Diagnose und Behandlung von Tumorerkrankungen zu verbessern. Das Konsortium ist mit vier KMU (inkl. des Konsortialführers) und vier Krankenhäusern (darunter drei Universitätskliniken) sehr ausgewogen und hervorragend besetzt. Zum Projektende sollen die folgenden Ziele erreicht werden: 1. Nachhaltige Stärkung der beteiligten KMU durch die Entwicklung neuer, einzigartiger Produkte und somit die Schaffung von Arbeitsplätzen im High-Tech Bereich. 2. Die universitären Zentren und Kliniken werden beim Ausbau ihrer akademischen Leistungsfähigkeit unterstützt, was wiederum zu neuen Zuschüssen und Fördermitteln führen wird. 3. Verbesserte Versorgung von Patienten mit Tumoren im Kopf-Hals Bereich, der Speiseröhre, der Lunge und der Leber durch die Einführung von Methoden der personalisierten - und gleichzeitig kostengünstigeren - Precision Medicine; 4. Schaffung eines "win-win" Netzwerks (in Bezug auf Patienten, KMU und akademischen Zentren), die ein verteiltes Lernen aus medizinischen Bilddaten ermöglicht und dadurch die Rentabilität des Projekts ermöglicht.

Expected Outputs (FR): Le présent projet permettra au moins trois résultats principaux :
1) Mettre à jour et/ou installer la solution d’analyse d’image distribuée dans tous les hôpitaux participant aux projet (voir animation : https://youtu.be/nQpqMIuHyOk );
2) Développer et valider un flux de travail (« workflow ») entre les participants dans l’ordre: analyses d’image médi-cales de manière distribuée (sans que les images quittent l’hôpital d’origine), création de signatures radiomiques utiles en clinique, validation eurorégionale des signatures, intégration dans des produits potentiellement commer-cialisables.
Ce nouveau flux de travail donnera naissance à « un cycle vertueux économique » avec :
1. Des produits commercialisables (voir point 3) ;
2. De nouvelles possibilités d'emploi dans l’Eurorégion ;
3. Des royalties (des redevances) destinées aux universités et hôpitaux (Figure 1) ainsi que des publications de revues scientifiques.
3) Tester Cinq types de produits potentiellement commercialisables au sein du réseau EURADIOMICS :
a) des signatures d’imagerie radiomiques potentiellement brevetable,
b) des logiciels d’analyse d’image,
c) des support décisionnels pour médecins,

Renforcement des PME eurorégionales existantes avec de nouveaux produits:
Les informations issues des analyses des mégadonnées provenant des institutions d’imagerie médicale transfrontalières participantes serviront, d’une part, à développer de nouveaux produits commercialisables (de 5 types différents : des signatures d’imagerie radiomiques brevetables, des logiciels d’analyse d’images, des support décision-nels pour les médecins et/ou les patients (« Patients Decision Aids ») et des modules d’e-apprentissage) et, d’autre part, à assurer la sécurisation de la propriété intellectuelle des institutions participantes. Il s'ensuit que chaque nou-veau produit émanant de ce projet pourra être lancé dans le monde entier. Le marché potentiel pour le logiciel de soutien de prise de soutien dans les soins médicaux est considérable.

2. Neue Beschäftigung in der Euregio;
3. Lizenzgebühren für Universitäten und Krankenhäuser (Abbildung 1) und Veröffentlichungen in wissenschaftlichen Zeitschriften.

die onwikkelen klinisch relevanten patentierbar radiomics sigantures, Entscheidungshilfen für Ärzte und / oder Patienten und E-Learning-Module), und zweitens, um das geistige Eigentum der beteiligten Institutionen zu gewährleisten. Daraus folgt, dass jedes neue Produkt dieses Projekts wird weltweit eingeführt werden. Der potenzielle Markt für die Entwicklung von Decision Support Software im Gesundheitswesen ist von Bedeutung.
