Objectif général : L’objectif du projet INTRAMES est d’améliorer à court terme la prise en charge des patients atteints de troubles de l’hydrodynamique cérébrale en développant conjointement un nouvel outil de diagnostic et d’investigation de ces altérations. Le second objectif est la compréhension générale du fonctionnement de l’hydrodynamique cérébrale en réalisant pour la première fois un fantôme multicompartimental (LCS, sang, cerveau) permettant d’étudier, in vitro, lors d’un test d’infusion, les interactions entre les écoulements du LCS et la PIC à partir d’une approche méthodologique partagée. Principales actions : - Mise en place à Cambridge de la méthodologie d’acquisition et d’analyse de l’IRM de flux développée à Amiens. - Développement d’un logiciel pour l’analyse combinée des résultats de l’IRM de flux et de la mesure de la PIC. - Conception et réalisation du fantôme. - Etude des relations fluides, tissus et pressions grâce au fantôme. - Exploitation du fantôme dans le cadre de la simulation d’un test d’infusion. - Etude de l’impact de la ventriculocisternostomie endoscopique sur l’hydrodynamique cérébrale avec la participation du service de neurochirurgie d’Amiens. - Exploitation et amélioration du modèle numérique (Cambridge) par introduction des données IRM de flux (Amiens).

Description (EN):

Main objective : The main objective of the INTRAMES project is to improve in the short-term the care of patients struck down by cerebral hydro-dynamical disorders, by jointly developing a new diagnosis and investigation tool for these disorders. The second objective lies in the global understanding of cerebral hydrodynamics. To reach this goal, INTRAMES plans to build the first multicompartmental (CSF, blood, brain) phantom to study in-vitro, during an infusion test, the interactions between CSF (cerebrospinal fluid) flow and ICP (intracranial pressure) starting from a shared methodological approach. Main actions : - Setting up in Cambridge of the MRI flow acquisition and analysis methods previously developed in Amiens. - Development of a software combining CSF and blood flow measurements resulting from MRI acquisition and direct measurements of intracranial pressure. - Design of the phantom - Study of the relationships between biological fluids, tissues and pressure via the phantom. - Utilization of the phantom within the framework of an infusion test simulation - Study of the impact of endoscopic ventriculocisternostomy on cerebral hydrodynamics with the participation of the neurosurgery department of Amiens - Exploitation and improvement of the numerical model (Cambridge) of CSF circulation using MRI measurements data (Amiens)

Principales réalisations / résultats : - 1 protocole d’acquisition IRM de flux, logiciel d’analyse IRM de flux, mesure flux&pic sur >20 patients réalisé pour l’acquisition flux et PIC CAMBRIDGE. - 1 Logiciel d’analyse combiné de l’IRM de flux et de la pic développé. - 1 plan d’exécution du fantôme et 1 banc expérimental complet. - Mesures flux sur >30 patients réalisées pour l’étude clinique. - 1 validation du modèle numérique in vivo. - 4 soumissions d’articles et de communication dans des congrès.
