Croatia and Serbia are still at the very beginning of the EU in the protection and provision of palliative care, where palliative medicine as a separate activity is not fully implemented in the health care system and the establishment of palliative care is one of the priorities of both countries.Given that the main aim of palliative care is to provide a quality and dignified life in final phase, the project approach is to provide health and social services for old, helpless, chronic patients and people with disabilities in the rural and underdeveloped program area of VSC and Mačva County. Rural areas are characterized by negative trends such as emigration, aging of the population and abandoned households, as well as poor infrastructural conditions associated with undeveloped transport accessibility and lack of appropriate public services. The project proposal is the logical sequence and extension of the previous project in which we have recognized the needs of the users, as well as the need to enhance the teams in the field that will use the acquired material resources, knowledge and skills. The project aims to improve the availability and quality of health /social services in rural areas for PWDs, old, helpless and chronic patients (sequel of mobile teams in Croatia, establishment of mobile teams in Serbia, palliative department in Home Ilok) to improve the basic skills of health/social workers through continuous professional training, the cooperation of existing partners' and the network expansion of new partners to improve the service delivery mechanism through the transfer of knowledge and experience, improve inter-sectoral colaboration with the aim of developing services and improving their quality. Main results of the project: 1) 640 people covered by health / social services; 100 health / social workers educated in the area of palliative and social care and palliative department in Home Ilok.

Achievements (EN):

A total of 123 health, social and other workers were educated through the project, and medical equipment and vehicle needed to provide health and social services were procured. Mobile teams for elderly people and persons with disabilities were organised and they provided health and social services for more than 740 people. One department in Home Ilok has been reconstructed and equipped with 10 beds for people in terminal phase of illness. Rooms are equipped with all necessary medical and other equipment to provide all necessary service and improve users’ quality. Total value of reconstruction, installation of hospital elevator and equipping of department is EUR 229.000.

Expected Results (EN): Number of elderly people and children assisted by social services provided through government bodies.

Expected Outputs (EN):

Expected results of the project are: 1. Performed activities according to the work plan and the Grant agreement: 2. Team members educated for work in the field of palliative and social care: 3. Integrated health/social services - mobile teams for the elderly, inffirm, cronic patients and PwDs. 4. Adapted and equipped palliative care department at Home Ilok for the elderly and illness in terminal stage of disease. 5. Increased visibility and position of vulnerable groups. Project partners identified challenges in border areas such as: lack of quality mechanisms for delivering health and social services to disadvantaged users. The project aim is to provide health and social services adapted to the needs of disadvantaged people so that they can stay longer in their homes, assist them in everyday life and health problems, and make health / social care more accessible and responsive to contemporary needs. The project improves palliative care, the level of palliative care workers' education, the establishment of new approaches, systematic co-operation of several institutions, cross-sector cooperation and flexibility. This multimodal and cross-sectoral approach directly contributes to the indicator of capacity mobilization program at different levels of care to achieve optimal accessibility and availability.
