Description (DE): In der Euregio Maas-Rhein (EMR) existieren derzeit wenige Konzepte zur Patientensicherheit (PS). Für die in der EMR mobilen Patienten existieren zu wenige Initiativen, die ihre Bedürfnisse adressieren. Vorhandene nationale Projekte in PS sind nicht aufeinander abgestimmt und differieren stark. Standardisierung und Harmonisierung von lokalen Initiativen würden die PS in der EMR erhöhen und so die Mobilität der Bürger verbessern. Ungleich dem wirtschaftlichen und sozialen Zusammenwachsen mangelt es an Bewusstsein und Austausch in Bereichen der PS zwischen den Gesundheitssystemen. Das Thema PS benötigt mehr Aufmerksamkeit und Verbesserungen in den einzelnen Ländern als auch gemeinsame Ansätze.
Risiken und Gefährdungen der Sicherheit eines Patienten resultieren häufig in vermeidbaren medizinischen Problemen, längeren Krankenhausaufenthalten und höheren Kosten. Eine Analyse der Weltgesundheitsorganisation (WHO) stellt fest: „…dass medizinische Fehler und unerwünschte Ereignisse in der Gesundheitsversorgung in 8-12% der Klinikaufenthalte auftreten. Die konsequente Verwendung von Strategien zur Reduzierung unerwünschter Ereignisse, würde allein in der EU zur Verhinderung von mehr als 750.000 schadenverursachender medizinischer Fehler pro Jahr und dadurch zu über 3.2 Millionen weniger Klinikaufenthaltstage führen. Die Anzahl Betroffener mit bleibender Einschränkung könnte um 26.0000 Fälle, die Anzahl der Todesfälle pro Jahr um 95.000 reduziert werden“[ [www.euro.who.int/en/health-topics/Health-systems/patient-safety].
Bezeichnend für eine Grenzregion in Europa sind die Aufnahme oder der Transfer eines Patienten zu einem nahegelegenen, aber im Ausland liegenden Arzt oder Krankenhaus. Dies ist ein kritischer Punkt, wenn es darum geht, die PS zu gewährleisten. Herausforderungen sind: Fragmentierte Gesundheitssysteme, die unverbunden nebeneinander stehen und Risiken für Patienten erhöhen.
Problematische Punkte bei Betrachtung von PS sind Aufnahme, Verlegung innerhalb oder zwischen Krankenhäusern und die Entlassung zu anderen Gesundheitsversorgern (z.B. Hausärzte) und umgekehrt. Laut Eurostat fanden im Jahr 2014 in der EU rund 130 Millionen Entlassungen aus Krankenhäusern statt, d.h. pro Stunde werden im Durchschnitt mehr als 14.000 Patienten aus europäischen Krankenhäusern entlassen [http://www.webcitation.org/6VygzGSP2]. Die Übertragung von Verantwortung für einen Patienten ist eine kritische Stelle, welche leicht Ursprung medizinischer Fehler, Verzögerungen bei der Diagnostik, lebensbedrohlicher Komplikationen, Unzufriedenheit der Patienten, höherer Kosten für das Gesundheitssystem und längerer Krankenhausaufenthalte ist [http://www.ncbi.nlm.nih.gov/pubmed/17327525].
Außerdem stellt die WHO fest, dass das Thema PS nicht zufriedenstellend unterstützt und verbreitet wird. Beispielsweise wird in den Bereichen Aus- und Weiterbildung, klinischer Praxis und Qualitätsmanagement, PS nur unzureichend adressiert. Die EMR ist mit ihren innovativen Institutionen, die sich im SafePat Konsortium vereinen, und Ansätzen zur Verbesserung von PS, bestens aufgestellt, um sich diesen Herausforderungen zu stellen.
Aktivitäten und Ergebnisse: Projektziele sind:
Analyse des Ist-Zustandes: Wissenschaftliche Analyse derzeitiger Prozesse zur Verbesserung von PS in Partnerländern um die „best practices“ und den Bedarf zu ermitteln.
Harmonisierung der Ansätze: Entwicklung einheitlicher Trainings- und Arbeitsabläufe (z.B. für medizinische Übergaben, Sicherheitschecklisten für Transfer, Entlassbriefe) medizinischer Versorger um sich gemeinsamen Standards zu nähern.
Empowerment von Patienten und medizinischem Personal: Informationsnetzwerk zur Aus- und Fortbildung von Patienten und medizinischem Personal um aktive Mitwirkung bei dem Thema PS zu generieren.
Entwicklung innovativer Tools: Entwicklung innovativer Technologien (z.B. Apps, digitale Checklisten und Datenbanken) um die Aufmerksamkeit, Standardisierung und Archivierung für verfügbare Informationen zu erhöhen und Abläufe zu unterstützen.
Entwicklung eines Akkreditierungs-Rahmenwerks: Akkreditierung von Anbietern in der Gesundheitsversorgung zu PS mit verschiedenen Graden (z.B. Bronze, Silber, Gold).
Gründung eines regionalen Netzwerkes für PS: Etablierung um Standardisierung, Schulungen, Kommunikation und Kooperationen zwischen den verschiedenen Akteuren zu koordinieren und erweitern.
Mehrwert für die grenzüberschreitende Zusammenarbeit: Wie andere Interrreg Projekte z. B. euPrevent bereits zeigen, ist die EMR ein perfektes Ökosystem um zu demonstrieren, wie effektive grenzüberschreitende Zusammenarbeit in Bezug auf Abläufe und Regelungen zur Verbesserung der PS in Europa umgesetzt werden können. SafePat kann den Grundstein in PS für andere Regionen und ganz Europa legen. Das Ergebnis wird eine Erhöhung der PS, eine Steigerung des Vertrauens in die grenzüberschreitende Gesundheitsversorgung, eine Reduktion der Kosten sein. Damit adressiert SafePat die Kernziele des INTERREG V-A Programms.

L´Euregio Meuse-Rhin (EMR) est caractérisée par une pénurie de directives et normes en matière de sécurité des patients. Les directives et normes disponibles en matière de sécurité des patients sont incohérentes et dépourvues d´interopérabilité. La normalisation et l´harmonisation des initiatives locales en matière de sécurité des patients stimulent l´uniformisation entre les établissements de santé régionaux, augmentant ultimement la sécurité des patients dans la région EMR entière.
Contrairement à la croissance industrielle et économique, la sensibilisation et la communication regardant les questions de sécurité des patients sont sous-développées, nécessitant d´urgence plus d´attention et une amélioration qualitative.
Les risques et dommages en matière de sécurité des patients entraînent souvent des problèmes de santé inévitables pour les patients, entraînant une augmentation des séjours à l´hôpital et des coûts supplémentaires pour le système de santé. Une analyse de la sécurité des patients de l´OMS a constaté que :
« L´utilisation conséquente de stratégies pour réduire le taux d´incidents dans la seule Union Européenne pourrait mener à la prévention de plus de 750 000 erreurs médicales causant des préjudices par an, entraînant plus de 3,2 million de jours d´hospitalisation, 260 000 incidents et 95 000 décès en moins par an. » [http://www.euro.who.int/en/health-topics/Health-systems/patient-safety/data-and-statistics].
Typiquement pour une région frontalière dans l´Union Européenne (UE), les patients ainsi que les fournisseurs de soins de santé sont également mis au défi par des systèmes de santé fragmentés et non liés, amplifiant encore les risques de sécurité des patients.
Les moments très critiques dans la chaine de sécurité des patients et en particulier de sécurité des patients transfrontaliers sont l´admission du patient à l´hôpital, le transfert dans ou entre les hôpitaux et la sortie de l´hôpital vers d´autres fournisseurs de soins de santé tels que des médecins de famille. Selon Eurostat, il y a eu 130 millions de sorties d´hôpital dans l´UE en 2014. Cela signifie qu´en moyenne plus de 14 000 patients sortent chaque heure des hôpitaux dans l´UE [http://www.webcitation.org/6VygzGSP2]. Le transfert de responsabilité pour les soins au patient est donc un événement très fréquent et important qui, s´il est mené de façon incorrecte, peut entraîner des erreurs médicales, un retard de diagnostic, des effets indésirables mettant la vie du patient en danger, l´insatisfaction du patient, des dépenses de santé augmentées, des durées d´hospitalisation augmentées, et d´autres effets qui ont un impact sur le système de santé [http://www.ncbi.nlm.nih.gov/pubmed/17327525]. En outre, l´OMS indique que la sécurité des patients dans différents domaines n´est pas établie et fournie par les systèmes de santé de manière satisfaisante. Par exemple, l´éducation, le traitement clinique et la gestion de qualité en matière de sécurité des patients sont mal pris en compte ou seulement partiellement implémentés.
L´EMR est en bonne position pour relever ces défis, avec quelques établissements hautement innovants et approches en matière de sécurité des patients qui sont rassemblés dans le consortium SafePat.
Objectifs: L´amélioration de la sécurité de patients en réduisant les risques associés avec l´admission, le transfert et la sortie d´hôpital de patients, à travers la connexion et la normalisation d´exemples de bonne pratique dans l"EMR.
Activités et résultats: Les objectifs du projet seront abordés sur 6 niveaux :
L´analyse de l´état de l´art : Analyse scientifique des systèmes actuels de sécurité des patients dans les pays partenaires pour identifier les bonnes pratiques et les nécessités locales.
Approches d´harmonisation : Développement de procédures de formation et de travail harmonisées (p. ex. transferts médicaux, listes de vérification de sécurité, lettres de congé) dans les hôpitaux et sites de soins ambulatoires pour atteindre une norme commune de sécurité de patients.
Developpement d´outils innovants : Développement de technologies de soutien innovantes pour augmenter la sensibilisation aux informations médicales disponibles et soutenir des procédures de sécurité des patients pour personnel médical et patients.
Responsabilisation des patients et du personnel médical : Donner du pouvoir aux patients et au personnel médical à travers de procédures d´entraînement et l´introduction d´outils pour augmenter la sensibilisation et les connaissances regardant la sécurité des patients.
Développement d´un cadre d´accréditation : Création d´un cadre d´accréditation/certification pour les fournisseurs de soins de santé en matière de sécurité des patients, avec différents niveaux.
Création d´un réseau régional sur la sécurité des patients : Établissement d´un réseau régional sur la sécurité des patients pour coordonner et établir la normalisation, la formation, la communication et la coopération entre les différents fournisseurs de soins de santé.
Valeur ajoutée de la collaboration transfrontalière: D´autres projets Interreg - tel que euPrevent [http://www.euprevent.eu] - l´ont déjà prouvé : l´EMR est un écosystème parfait pour démontrer avec quelle efficacité des procédures et politiques transfrontalières de sécurité des patients peuvent être menées dans le 21e siècle en Europe. Outre les projets Interreg antérieurs et précédents concernant la prévention sanitaire et la mobilité des patients, SafePat se concentrera sur la qualification, la responsabilisation et la formation, les procédures normalisées et la communication pour améliorer la sécurité des patients dans l´EMR. Le projet SafePat s´efforcera de devenir la base pour d´autres régions frontalières et pour l´Europe en général. Les résultats seront une augmentation du niveau de sécurité des patients et une réduction des risques de préjudices aux patients, une augmentation de la confiance dans les systèmes de santé transfrontaliers, une réduction des coûts à travers de procédures normalisées, et un ensemble d´outils hautement innovants pour les professionnels de la santé et les patients. Read more Description (EN): The overall aim of the SafePat project is to increase the wellbeing of citizens concerning patient safety within the Euregio Maas-Rhein.
Challenges: The Euregio Maas-Rhein (EMR) is characterized by a scarcity of patient safety guidelines and standards. The available guidelines and standards on patient safety are incoherent and lack interoperability. Standardisation and harmonisation of local patient safety initiatives, stimulate uniformisation between regional healthcare institutions ultimately increasing patient safety in the entire EMR region.
In contrast to the industrial and economical growth, awareness and communication on patient safety issues is underdeveloped urgently requiring more attention and qualitative upgrading.
Risks and harms in terms of patient safety often result in evitable health problems for patients resulting in an increased hospital stay and an added cost to the healthcare system. A patient safety analysis of the WHO identified that:

Typical for a border-region in the European Union (EU), patients as well as healthcare providers are additionally challenged by fragmented and unconnected healthcare systems even amplifying the patient safety risks.
Highly critical moments in the chain of patient safety and especially cross-border patient safety are the patient’s admission to hospital, transfer within or between hospitals, and discharge from the hospital to other healthcare providers like family doctors. According to Eurostat there were 130 million discharges in the EU in 2014. This means that on average more than 14000 patients are discharged every single hour from hospitals in the EU. The transfer of responsibility for the patient’s care is therefore very frequent and important occurrences that if improperly conducted, can lead to medical errors, delay in diagnosis, life threatening adverse events, patient dissatisfaction, increased healthcare expenditure, increased length of hospital stay, and other effects that impact on the healthcare system. In addition, the WHO states that patient safety in different fields is not satisfactorily established and provided by the healthcare systems. For example education, clinical treatment and quality management in patient safety are poorly addressed or only partially implemented.
The EMR is well positioned to address this challenge with some highly innovative institutions and approaches towards patient safety that are combined in the SafePat consortium.
Objectives: Improving patient safety, by reducing risks associated with patient admission, transfer and discharge, through connecting and standardising best practices examples in the EMR.
Activities and outcomes: The project objective will be addressed on 6 levels:
Analysing the state-of-the-art: Scientific analysis of the current patient safety systems in the partner countries to identify best practices and local needs.
Harmonising approaches: Develop harmonised training and working procedures (e.g. medical handovers, safety checklists, discharge letters) at hospital and ambulatory care sites to reach a joint standard of patient safety.
Developing innovative tools: Developing innovative support technologies to increase awareness of available medical information and support patient safety procedures for medical staff and patients.
Empowering patients and medical staff: Empowering patients and medical staff by training procedures and introducing tools to become more aware and knowledgeable about patient safety.
Developing an accreditation framework: Creating an accreditation/certification framework for healthcare providers on patient safety with different levels.
Founding a regional network on patient safety: Establishing a regional network for patient safety to coordinate and establish the standardisation, training, communication and cooperation between the different healthcare providers.
Added value of cross-border collaboration: Other interreg projects - like euPrevent - already proved: the EMR is a perfect eco-system to demonstrate how effective cross-border patient safety procedures and policies can be conducted in the 21st century in Europe. In addition to previous and preceding interreg projects concerned with health prevention and patients’ mobility, SafePat will focus on qualification, empowerment and training, standardised procedures and communication to improve patient safety within the EMR. The SafePat project will strive to become a foundation for other border regions and Europe at large. The results will be an increased level of patient safety and a reduction in the risks of harm to patients, an increased trust in cross-border healthcare systems, reduced cost by establishing standardised procedures, and a set of highly innovative tools for medical professionals and patients. SafePat therefore addresses the core objectives of the INTERREG V Programme objective 4: Territorial Development.

End users blended curriculum on SBAR (Situation, Background, Assessment, Recommendation):
Interactive e-learning This intervention first foresees a 45 minutes interactive e-learning session. At the end of the e-learning, participants are invited to complete a multiple-choice questionnaire to assess their knowledge. Ten questions are randomly generated by the system from a database. A score of 80 percent is required for learners to pass the course. Each participant has two attempts to reach 80 percent of correct answers. (Related link: SafePAT Massive Online Open Course on SBAR, UHeerlen, 2020.)
Face-to-face training The second part of the intervention entails a face-to-face training dedicated to the SBAR end users. This is a 90 minutes class with a short theoretical reminder (10 minutes) followed by practical exercises based on clinical simulations. Each simulation is followed by a debriefing conducted upon the Center for Medical Simulation (CMS) model.

Empowering medical and nursing students in the Euregio Meuse-Rhine:
Hasselt University and nursing students in University College Leuven-Limburg were empowered in their patient safety competences during hands-on sessions on patient safety.
Our repeated cross-sectional study on self-perceived patient safety competences in medical and nursing students demonstrated that confidence with regard to teamwork skills is lacking in two out of three students. In spite of the blame-free culture, which has been advocated by patient safety experts for several decades, 70% of students still fear disciplinary action after making a serious error. Therefore it is important for students to realise that errors (can) occur every day, and that it is vital to disclose adverse events and to seize the opportunity to turn them into a learning opportunity.
